DHS requests public comments on acute care transitions

DHS requests public comments on DHS – Acute Care Transitions Advisory Council Report and Recommendations (PDF). We request feedback on barriers and potential solutions to help people move from acute care settings to nonacute care settings.

Process to submit comments

Email comments to TransitionsAdvisoryCouncil.DHS@state.mn.us no later than 4 p.m. Wednesday, Oct. 30, 2024.

Background information

In October 2023, DHS formed the Acute Care Transitions Advisory Council (ACTAC) to advise and help in implementing a unified statewide vision and systemic approach to address acute care transitions in Minnesota. For more information, refer to:
