Connecting families project grant opportunity

DHS is accepting applications from family support organizations for a grant opportunity to provide support to Minnesota families of children with disabilities through individual and group support services, resource connections and Charting the LifeCourse (CtLC) education.

Proposals should meet one or both of the following goals:

  • · Family Connection and Support: Provide individual and group support and link families to essential resources to reduce isolation.
  • · Empowering Families with Charting the LifeCourse (CtLC): Introduce and guide families and professionals in using the CtLC framework and tools to envision a good life, identify supports and engage in meaningful planning.

    Use the following information to apply:

  • · Deadline: 4 p.m. on Monday, March 9, 2026
  • · Application: To apply, refer to DHS – Open grants, RFPs and RFIs – DHS is seeking to provide support services to families of children with disabilities.
  • · Number of grantees: 4-5
  • · Contract length: DHS will issue one-year grant contracts and may extend contracts for up to four years.
  • · Funding amount for Family Connection and Support: Up to $93,000
  • · Funding amount for Empowering Families through CtLC: Up to $30,000
  • Note: Applicants may apply for one or both grant opportunities.
