Purpose: To remind EIDBI agencies to apply for a provisional license in the DHS Provider Hub by May 31, 2026, and announce two online information sessions about provisional licensure

Reminder: EIDBI provisional license applications due May 31, 2026
All EIDBI agencies must submit a provisional license application in the DHS Provider Hub by May 31, 2026.

How to apply
To submit a provisional license application through the DHS Provider Hub:
1. The EIDBI agency chooses one of its controlling individuals to be its authorized agent, as defined in Minn. Stat. §245A.02, subd. 3(b). For more information about the authorized agent’s role, refer to page two of Pre-Licensing Application Checklist, DHS-8818 (PDF).
2. The authorized agent creates a DHS Provider Hub account at DHS Provider Hub – Login.
3. The authorized agent uses their account to complete the agency’s provisional license application.
