Night supervision defect in MnCHOICES
DHS is working to resolve a night supervision defect in MnCHOICES. MnCHOICES users reported an error message that does not allow them to authorize or save changes unless they reduce the total number of night supervision units to no more than 48 for the service span.
MnCHOICES user instructions for service start dates of April 1, 2026, or later
MNCHOICES users must:

2. Authorize night supervision services with start dates of April 1, 2026, or later with the following service codes and modifiers:
3. Add night supervision to service agreements in the Medicaid Management Information System (MMIS) under S5135 UA with the rate that populated in MnCHOICES with start dates of April 1, 2026, or later.
Note: Enter the staffing ratio and asleep/awake modifiers in the line description in MMIS.
MnCHOICES user instructions for service start dates from Jan. 1, 2026, to March 31, 2026
MnCHOICES users must continue to follow the rate remediation instructions to use the amended framework to manually calculate the rate for night supervision service start dates between Jan. 1, 2026, and March 31, 2026. DHS emailed these instructions to Disability Waiver Rate System (DWRS) rate mentors on April 24, 2026.
Note: Line description notes are not required in MMIS for night supervision with service start dates before April 1, 2026.
