  • · We removed content for oral surgery, orthodontics, Temporomandibular Joint (TMJ) Disorder and pain relief services.

  • · MHCP Member Evidence of Coverage – Effective on or after Jan. 1, 2024, the Minnesota Legislature removed the copay and family deductible for all Medical Assistance members. Effective on or after Jan. 1, 2024, the Minnesota Legislature also changed several MinnesotaCare copay fees and removed the family deductible.

  • · Under Eligible Providers, Provider Responsibilites, we added services must be delivered and documented for two hours per encounter. We also clarified the level of care screening tool to be used to determine if services are medically necessary.

  • · Under Covered Services, we clarified information about receiving the daily encounter rate.

D4910 can be performed once every 91 days for 730 days following a submitted claim to the Minnesota Department of Human Services with a paid service line for CDT code D4341 or D4342. After 24 months, D4910 is not payable unless D4341 or D4342 are performed again under a new prior authorization.
