MN–ITS User Manual

Submit a Health Information Request (HIR)

Heath Information Request (HIR) is a clinical tool; it allows you to search up to three years of claims history for inpatient and emergency department history and up to one year of drug utilization history about a Minnesota Health Care Programs (MHCP) member. HIR is designed to be used by pharmacies and individual clinicians registered for MN–ITS within one of three MHCP-enrolled provider type organizations: primary care clinics, hospitals or pharmacies.

Using MN–ITS Interactive

  • · Log in to MN–ITS
  • · Select Health Information Request from the left-hand menu
  • Submitting a Health Information Request

  • · Complete all bolded and asterisked (required) fields. Field titles with a single red asterisk (*) indicate that the information is mandatory.
  • · Enter the Subscriber ID (8-digit Minnesota Health Care Programs number).
  • · From Date and To Date default to one year from today’s date. Adjust dates as needed.
  • · Select Continue
  • Reviewing the Health Information Response

    If a match is found within any of the three claims history criteria (Emergency Department, Inpatient, or Pharmacy), the Subscriber (member) Name and information will display in the subscriber information screen.

  • · Select the appropriate tab to review the data displayed.
  • · From Date and To Date can be adjusted using the Refresh button.
  • · Use the Search function in the upper right corner to narrow the display results by using specific criteria.
  • · Use the up and down arrows to the right of each column header to sort the data.
  • · If there is information for more than 10 records displayed, navigate to the next screen by clicking NEXT on the lower right corner.
  • · [Print Page] on the top right corner allows you to print the Response page.
  • · < Back to Search Page will return to the initial Health Information Request screen
  • Emergency Department

    For Emergency Department records, you can enter the "From" date only dating back to THREE years from today's date.

    If no data is available, you will receive the following response: No Emergency Dept. records exist for this date range. Inpatient or Pharmacy records may exist.

  • · Select either the diagnosis code or the diagnosis code description to display the Emergency Department visit details
  • · Print the visit details using the [Print Page] function
  • · < Back to Results Page will return to the initial Health Information Request screen
  • Inpatient

    For Inpatient records, you can enter the "From" date only dating back to THREE years from today's date. Inpatient record results do not include SUD treatment.

    If no data is available, you will receive the following response: No Inpatient records exist for this date range. Emergency Dept. or Pharmacy records may exist.

  • · Select either the diagnosis code or the diagnosis code description to display the Inpatient visit details
  • · Print the visit details using the [Print Page] function
  • · < Back to Results Page will return to the initial Health Information Request screen
  • Pharmacy

    For Pharmacy records, you can enter the "From" date only dating back to ONE year from today's date.

    If no data is available, you will receive the following response: No Pharmacy records exist for this date range. Emergency Dept. or Inpatient records may exist.
