5.1.0. Client Status Record

Client Status Record

The client status record is an unduplicated data record within a single provider agency. This means each record corresponds to one person who has received services at the time of the status update. Select the data values that apply to the client at the time of the status update.

Client status records are completed when a client begins services, every six-months that they continue to receive services and at discontinuance of service. In addition, client status records should be updated if a client has a change in funding source, and/or if a client begins a new program within the reporting period.
