MN–ITS User Manual

Provider Lists

Revised: August 7, 2025

Provider Lists are downloadable Excel spreadsheets made available to MN–ITS users and contain Minnesota Health Care Programs (MHCP)-enrolled provider information.

Steps to view the Provider Lists in MN–ITS:

  • 1. Log into MN–ITS. You must be enrolled with MHCP and have a registered MN–ITS mailbox.
  • 2. On the left-side navigation, select Provider Lists (Individual PCAs) to review the Personal Care Assistance (PCA) Providers A - Z list or select Provider Lists and select one of the following lists from the middle of the screen:

  • · FQHC/RHC Carve Out List
  • · Health Care Homes
  • · ICF/DD Providers (for Day Training and Habilitation (DT&H) Day Services Claims)
  • · Moving Home Minnesota
  • · Ordering/Prescribing Providers
  • · Practitioner Affiliations A - Z
  • · Referring Providers
  • · Telemedicine/Telephonic Telemedicine Providers
  • Providers can use the Minnesota Provider Screening and Enrollment (MPSE) portal to review and make affiliation requests.

    FQHC/RHC Carve Out List

    List is used by managed care organizations (MCOs) and identifies MHCP-enrolled federally qualified health centers (FQHC) and rural health clinics (RHC) for the managed care carve-out claims process. MCOs can use the list to identify if the FQHC or RHC provides medical, dental or pharmacy services.

    Columns C – E identifies if medical services, dental services, or pharmacy services are provided. An entry of Y stands for ‘yes’ and an entry of N stands for ‘no’. An entry of Y will also be highlighted in green.

    Column O displays the FQHC or RHC carve-out status begin date.

    Column P displays the FQHC or RHC carve-out status end date, or if 12/31/3001 is displayed, the status does not expire.

    The list is updated monthly.

    Health Care Homes

    List of MHCP-enrolled hospitals, physicians (individuals or clinics), nurse practitioners and physician assistants who have been certified to be health care home providers.

    Column H displays the date the individual or organization was affiliated as a health care home provider.

    The list is updated weekly.

    ICF/DD Providers (for DT&H Claims)

    List of all MCHP-enrolled and active intermediate care facility for persons with developmental disabilities (ICF/DD).

    The list is updated monthly.

    Moving Home Minnesota

    List of all MHCP-enrolled and active waiver providers who provide Moving Home Minnesota services.

    The list is updated weekly.

    Ordering/Prescribing Providers

    List of all MHCP-enrolled providers who can order or prescribe services. These providers include physicians, dentists, optometrists, nurse practitioners, nurse midwives, clinical nurse specialists and physician assistants.

    Column K displays the specialty code. Specialty code PN will display when the services were provided under COVID-19 policies which have ended.

    Column N displays the enhanced services begin date.

    Column O displays the enhanced services end date. If the date displays 12/31/3001, the status does not expire except those with specialty code (column K) PN.

    The list is updated monthly.

    Personal Care Assistance (PCA) Providers A – Z

    List of all MHCP-enrolled and affiliated individual PCA workers and direct support workers (DSW).

    The list is used to verify:

  • · an individual PCA worker’s or DSW’s affiliation with a PCA organization or Financial Management Services (FMS) organization, and
  • · the individual PCA worker’s or DSW’s qualification for the enhanced rate, and
  • · the individual PCA worker has completed the Personal Care Assistance/Community First Services and Supports (PCA/CFSS) support worker training
  • Agency Affiliation
    Take the following steps to verify a worker is affiliated with a PCA or FMS organization.

    Steps to view data in MN–ITS:

  • 1. Log into MN–ITS. You must be enrolled with MHCP and have a registered MN–ITS mailbox.
  • 2. Select Provider Lists (Individual PCAs) on the left-side navigation.
  • 3. Select Personal Care Assistance (PCA) Providers A-Z from the middle of the screen.
  • 4. The data will open in Excel format with columns A-E showing affiliation data.
  • · Column A – Unique Minnesota Provider Identifier (UMPI): Assigned to direct support worker
  • · Column B – Provider name: Direct support worker name
  • · Column C – Organization name: PCA, CFSS or FMS organizations affiliated with the individual provider
  • · Column D – Provider group begin date: Date the worker became affiliated with the organization
  • · Column E – Provider group end date: Date the workers affiliation ended or is set to end, or if 12/31/3001 is displayed, the worker remains affiliated with the agency.
  • Enhanced Rate
    Take the following steps to verify that a worker is qualified for the enhanced rate.

    Steps to view data in MN–ITS:

  • 1. Log into MN–ITS. You must be enrolled with MHCP and have a registered MN–ITS mailbox.
  • 2. Select Provider Lists (Individual PCAs) on the left-side navigation.
  • 3. Select Personal Care Assistance (PCA) Providers A-Z from the middle of the screen.
  • 4. The data will open in Excel format with columns F, G and H showing the enhanced rate or CFSS training completed.
  • · Column F – License Type
  • · “P2 – Enhanced Rate” will display if the worker has completed the enhanced rate training. Member also needs to be approved for complex services.
  • · “DW-CFSS Training Completed” will display if the worker has completed PCA/CFSS support worker training. Required for all PCA/CFSS staff as of April 15, 2020.
  • · Note: If they have both you will see the individual listed twice for your agency.
  • · Column G – License Begin Date: Date the worker qualifies for the enhanced rate or completed the PCA/CFSS support worker training.
  • · Column H – License End Date
  • · Date the enhanced rate qualification has ended or is set to end
  • · Date the PCA/CFSS support worker training has ended or is set to end. If 12/31/3001 is displayed, the worker’s training remains valid.
  • If you notice enhanced rate training data is not showing for your worker but the worker has reported taking the course to be qualified, submit the Individual DSW Information Change Form (DHS-5716) (PDF). Check the box titled “DSW completed the Qualified Enhanced Rate training” and fill in the training completion date and training expiration date (if applicable). Also include the workers’ completion of training confirmation letter.

    A Provider Eligibility and Compliance specialist will review the DHS-5716 and, if applicable, add to the worker’s record. You will not receive a confirmation notice after the record has been updated. Use the Personal Care Assistance (PCA) Providers A – Z spreadsheet in MN–ITS to confirm the enhanced rate training is added to the worker’s record.

    If you notice your worker is not on this list, allow 30 days from the date you submitted your worker's enrollment application for processing. If you have not received your affiliation letter after 30 days in your PRVLTR folder in your MN–ITS mailbox, contact the MHCP Provider Resource Center.

    Tiered Wage PCA/CFSS

    Use the “Tiered Wage PCA/CFSS” list to find which wage tier a direct support worker belongs based on the hours of PCA or CFSS services a worker has provided since July 1, 2017. Take the following steps to view which tier direct support workers are in:

  • 1. Log into MN–ITS. You must be enrolled with MHCP and have a registered MN–ITS mailbox.
  • 2. Select Provider Lists (Individual PCAs) on the left-side navigation.
  • 3. Select Tiered Wage PCA/CFSS from the middle of the screen.
  • 4. The data will open in Excel format with columns A-I showing the following information:
  • · Column A – Unique Minnesota provider identifier (UMPI): assigned to direct support worker
  • · Column B – Provider Name: Direct support worker name
  • · Column C – Organization Name: PCA, CFSS or FMS Agencies affiliated with the direct support worker
  • · Column D – Affiliation Begin Date: Date the direct support worker became affiliated with agency
  • · Column E – Tier
  • · This cell will be blank if the provider has provided 0 to 1,000 hours of service since July 1, 2017.
  • · L1 - Tier 1 – 1,001 to 2,000 hours of service provided since July 1, 2017
  • · L2 - Tier 2 – 2,001 to 6,000 hours of service provided since July 1, 2017
  • · L3 - Tier 3 – 6,001 to 10,000 hours of service provided since July 1, 2017
  • · L4 - Tier 4 – 10,001 hours or more of service provided since July 1, 2017
  • · Column F – The Tier Effective Date: Is the date the individual has provided the required number of hours to fall in the tier listed in Column E.
  • · Column G – License Type: used to show the enhanced rate or PCA/CFSS support worker training completed.
  • · “P2 – Enhanced Rate” will display if the worker has completed the enhanced rate training. Member also needs to be approved for complex services.
  • · “DW-CFSS Training Completed” will display if the worker has completed PCA/CFSS support workers training. Required for all PCA/CFSS staff as of April 15, 2020.
  • · Note: If the DSW has the enhanced rate and CFSS training you will see the individual listed twice for your agency.
  • · Column H – License Begin Date: Date the worker qualifies for the enhanced rate or completed the CFSS worker training.
  • · Column I – License End Date
  • · Date the enhanced rate qualification has ended or is set to end.
  • · Date the PCA/CFSS support worker training has ended or is set to end. If 12/31/3001 is displayed the worker’s training remains valid.
  • If you notice enhanced rate training data is not showing for your worker but the worker has reported taking the course to be qualified, submit the Individual DSW Information Change Form (DHS-5716) (PDF).     Practitioner Affiliations A – Z

    List of all MHCP-enrolled individual providers who are affiliated with an organization except individual PCA workers and direct support workers.

    Column G displays the individual’s affiliation date with the organization.

    Column H displays the individual’s affiliation end date with the organization. If the date displays 12/31/3001, the affiliation does not expire.

    The list is updated weekly.

    Referring Providers

    List of all MHCP-enrolled and active individual providers except individual PCA workers and direct support workers.

    Referring providers are also known as rendering or treating providers.

    The list is updated weekly.

    Telemedicine/Telephonic Telemedicine Providers

    List of all MHCP-enrolled and active providers who can provide services via telemedicine, also known as telehealth.

    The list is updated weekly.
