Reporting enrollment changes, reinstitutionalization and disenrollment for Moving Home Minnesota (MHM)

Definitions

Disenrollment: A person’s participation in MHM has ended.

Suspension: A person’s participation in MHM has been paused temporarily, but the person is not disenrolled. Suspension only occurs when someone is reinstitutionalized for 30 days or more.

Reinstitutionalization: A person returns to an institutional setting for short-term or long-term care.

Short-term stay: A person’s inpatient stay of 1-20 days.

Medium-term stay: A person’s inpatient stay of 21-100 days.

Long-term stay: A person’s inpatient stay of 101 or more days.

Enrollment changes

The transition coordinator, licensed alcohol and drug counselor (LADC), MHM demonstration case manager, waiver case manager or care coordinator must use MHM Communication Form, DHS-6759H to notify DHS about changes to a person receiving MHM services during and after the transition to the community.

Changes during transition

Examples include when a person:

  • · Has a change in provider or case manager.
  • · Chooses not to use MHM services after they have been approved and enrolled in MHM.
  • · Decides they want to remain at the facility.
  • · Transfers to a hospital or other institutional setting.
  • · Passes away.
  • · Leaves the facility against medical advice and does not return.
  • · Cannot find housing.
  • · Moves to a non-qualified community residence.
  • · Needs an extension of transitional services.
  • Changes post-transition

    Examples include when a person:

  • · Has a change in provider or case manager.
  • · Chooses not to use MHM services after they have been approved and enrolled in MHM.
  • · Returns to an institutional setting for a short-, medium- or long-term stay.
  • · Passes away.
  • · Will not maintain Medical Assistance (MA) eligibility. The person must be on MA while receiving MHM services in the community.
  • · Relocates or moves to another state.
  • · Completes the 365-day MHM demonstration period (or 366-day period during a leap year).
  • Reinstitutionalization during MHM participation

    A person receiving MHM in the community may need to return to an institution (e.g., hospital, nursing facility, institution for mental diseases) for a short-, medium- or long-term stay. In these situations, the transition coordinator, demonstration case manager, waiver case manager or care coordinator must use MHM Communication Form, DHS-6759H to notify DHS about the change. They must complete this form, even if the reinstitutionalization is for one day.

    A person’s ongoing MHM participation may be affected depending on their length of stay at the institutional setting:

  • · 30 or fewer days: The person’s MHM participation and eligibility spans will not be interrupted.
  • · More than 30 days: The person’s MHM participation will be suspended effective the 30th day of the institutional stay, but they may use any time left on their 365/366-day MHM eligibility span after they return to their qualified community-based residence.
  • The person may also reapply for another full 180 days of transition planning and 365/366 days of MHM community services if they stay in the institutional setting for 60 or more days and meet the MHM eligibility criteria again.

    Disenrollment from MHM

    If a person disenrolls from MHM for any reason, the transition coordinator, LADC, demonstration case manager, waiver case manager or care coordinator must use MHM Communication Form, DHS-6759H to notify DHS about the disenrollment.

    Once a person is disenrolled, they cannot reapply for MHM services unless they meet the MHM eligibility criteria again.

    Disenrollment at the end of the 365/366-day eligibility period

    When a person receiving MHM moves into a qualified community-based residence, their move-in date is the start date for their 365/366 days of MHM community-based services eligibility. After the person’s 365/366-day participation period is complete, they must disenroll from MHM.

    Disenrollment before the end of the 365/366-day eligibility period

    A person can disenroll from MHM at any time before the end of their 365/366-day eligibility period. Disenrollment reasons include when a person:

  • · No longer wants to be enrolled in MHM.
  • · Decides they want to return to a facility.
  • · Passes away.
  • · Will not maintain MA eligibility; the person must be on MA while receiving MHM services in the community.
  • · Relocates or moves to another state.
  • · Moves to a non-qualified residence.
