Definitions

Moving Home Minnesota (MHM) home and community-based services (HCBS) coordinator: Professional who helps with intake and confirms a person’s eligibility to participate in the demonstration.

Lead agency: County of financial responsibility (CFR), tribal nation or managed care organization (MCO).

Eligibility

Transition planning, coordination and services

To enroll in MHM and receive 180 days of transition planning, coordination and services, the person must meet all of the following criteria:

  • · Currently residing in one of the qualified institutions
  • · Enrolled in Medical Assistance (MA) before discharge from the qualified institution and maintain enrollment in MA during the time span they are eligible to receive MHM services
  • · Have continuously resided in one or more qualified institutions for 60 or more consecutive days.
    Example: A person may start their institutional stay in a hospital, and then move to a facility providing rehabilitative services paid by Medicare, and then move to a nursing facility paid by Medicaid (i.e., MA). This person’s stay is continuous and fulfills the 60-day institutional stay requirement.
  • Services in the community

    To receive 365 days of MHM services in the community after the transition, the person must:

  • · Transition to one of the qualified community residences
  • · Meet an institutional level of care, as described on CBSM – Level of care
  • · Maintain MA eligibility.
  • MHM services for people age 65 and older

    All people age 65 or older who receive MHM must enroll in the Elderly Waiver (EW) to access MHM services, unless they are eligible to enroll in a disability waiver program. For additional information, see:

  • Qualified institutions

    A qualified institution can be any of the following:

  • · Child and adolescent behavioral health hospital (CABHH) in Willmar, MN
  • · Community behavioral health hospital (CBHH)
  • · Hospital
  • · Institution for mental diseases (IMD), with limitations (see note about IMDs)
  • · Intermediate care facility for persons with developmental disabilities (ICF/DD)
  • · Nursing facility
  • · Psychiatric residential treatment facility (PRTF) for children and adolescents.
  • Note about IMDs

    A person who resides in an IMD is eligible for MHM to the extent the services are covered by federally funded MA, as referenced in Eligibility Policy Manual – Program for people living in IMDs (section 2.5.4).

    An IMD is not a qualified institution for a person age 21-64, with the following exceptions:

  • · Days spent in the Anoka Metro Regional Treatment Center (AMRTC) or another IMD may be included in calculating the 60 consecutive days for a person age 21-64 if they are discharged from another qualified institution
  • · An IMD enrolled in the 1115 Substance Use Disorder (SUD) System Reform Demonstration is a qualified institution for people of any age.
  • Qualified community residences

    People enrolled in MHM must transition to a qualified community residence to maintain eligibility to receive MHM services in the community. A qualified community residence should honor personal choice and control of the person’s home and give them opportunities for independence and community integration.

    A qualified community residence includes:

  • · A home owned or leased by the person or their family member
  • · An apartment with an individual lease and living areas over which the person or their family has control
  • · An assisted-living residence that provides an apartment with separate living, sleeping, bathing and cooking areas, lockable entrance and exit doors
  • · A home in a residential setting in which no more than four unrelated people live.
  • Enrollment

    To enroll an eligible person in MHM, anyone can complete the online MHM Intake Form, DHS-5032. The intake form can be faxed to 651-431-7745 or mailed to:

    The Senior LinkAge Line (800-333-2433), Disability Hub MN (866-333-2466) or the person’s county or tribal human services agency are available to help provide information on assistance with enrollment in MHM.

    Upon receipt of the intake form, an MHM HCBS coordinator will confirm the person’s eligibility and work with their lead agency to facilitate the assignment of an MHM transition coordinator to begin the transition process.
