Comparison of MHM, RSC-TCM and HSS

Moving Home Minnesota (MHM), relocation service coordination – targeted case management (RSC-TCM) and Housing Stabilization Services (HSS) can all help a person on Medical Assistance (MA) move to the community. However, each program/service has different requirements for who it can serve.

A person has the right to choose which program/service is right for them, but sometimes it can be hard to determine which option they should use. Lead agencies should consider asking some of the following questions:

  • · Will the person have been in an institution for at least 60 days by the time they are ready to discharge? If so, MHM might be the best option to meet their needs.
  • · Can the person move out of a nursing facility sooner than 60 days? If so, RSC-TCM or HSS might be the best option to meet their needs.
  • · Where is the person moving from?
  • · What type of community residence does the person want to move to?
  • · What is the person’s level of care?
  • · Does the person need additional services to help them live in the community?

    Note: The person must meet the housing instability requirement by having one of the risk factors on MHCP Provider Manual – HSS eligible members.

    Level of care requirement

    The person must continue to meet an institutional level of care, as determined by a MnCHOICES or long-term care consultation (LTCC) assessment, to receive MHM community services post-transition.

    The person needs an assessment to determine and document their eligibility for RSC-TCM. The timeline for authorization is different depending on the type of assessment:

  • · If the person had a MnCHOICES or LTCC assessment, the lead agency can authorize RSC-TCM within 365 days from the date of the assessment.
  • · If the person had a developmental disabilities (DD) screening, the lead agency can authorize RSC-TCM within six months from the date of the assessment, as long as RSC-TCM is listed as a current service.
  • The person must have a documented disability or disabling condition and be assessed to require assistance. For more information, see MHCP Provider Manual – HSS eligible members.

    Community housing requirements

    The person’s home must meet one of the following criteria:

  • · A home owned or leased by the person or their family member.
  • · An apartment with an individual lease and living areas over which the person or their family has control.
  • · An assisted living residence that provides an apartment with separate living, sleeping, bathing and cooking areas, lockable entrance and exit doors.
  • · A home in a residential setting in which no more than four unrelated people live (including the person receiving MHM).
  • The person must transition to a community setting that meets the HCBS settings rule. For more information, see DHS – HCBS settings transition plan.

    The person must transition to a community setting that meets the HCBS settings rule, excluding foster care settings.

    The person cannot use housing transition services to move to a provider-controlled setting in which Brain Injury (BI) or Community Access for Disability Inclusion (CADI) 24-hour customized living services will support the person once they move in, unless the person is moving out of homelessness and also meets the definition of long-term homeless.     Additional services

    MHM demonstration services help the person remain in the community. These services include funds for furnishings, supplies, rent, utility deposits and moving expenses. For more information, see MHM demonstration and supplemental services table (PDF).

    There are no additional services.

    To access additional services after the 180 days of RSC-TCM expire, the person must enroll in an HCBS waiver, state plan services or MHM.

    HSS housing sustaining services help the person remain in the community. For more information, see DHS – HSS.

    Length of service eligibility

    The person receives 180 days of transition planning and coordination.

    MHM may grant extensions to the 180-day transition planning and transition coordination eligibility span under certain circumstances. A transition coordinator may request an extension by submitting MHM Communication Form, DHS-6759H and selecting the transition coordination extension request option from the reason for communication drop-down menu.

    The person receives 365 days of demonstration and community services after they move to the community.

    MHM community services can be suspended if the person is re-institutionalized for 30 days or more and then resume once the person returns to the community.

    The person receives 180 days of RSC-TCM.

    The person receives 150 hours of housing transition Services. DHS may authorize an additional 150 hours for people who meet exception requirements outlined in HSS Eligibility Request, DHS-7948.

    The person receives 150 hours of housing sustaining services. DHS may authorize an additional 150 hours for people who meet exception requirements outlined in HSS Eligibility Request, DHS-7948.

    Billing

    Providers may bill for the following services while the person is still in the institution:

  • · Transition plan development.
  • · Transition coordination.
  • · Transition services.
  • · Costs for finding housing and employment.
  • Providers must bill for post-transition demonstration and supplemental services after the person has moved to the community.

    Providers may bill for RSC-TCM while the person is in the institution.

    If the person is accessing HCBS waiver services, providers cannot bill until the person is opened to the waiver.

    For a person residing in an institutional setting, providers may only bill for housing consultation services and housing transition services provided during the time the person was in an institution after the date the person is discharged from the institutional setting and enrolled in housing transition services.
