Navigator program

The Navigator program seeks to break down barriers to health care coverage for eligible Minnesotans. Community organizations throughout the state are partnering with DHS and MNsure to provide free face-to-face help to people applying for and enrolling in health care coverage through MNsure. People may get help from any navigator listed in the assister directory.

Shorter renewal form streamlines paperwork for many clients

A shorter annual renewal form is being sent to certain populations monthly, beginning in July for renewals due for September 2012. The Combined Annual Renewal for Certain Populations, DHS-3727 (PDF) will be sent to seniors and people with disabilities in households of one or a married couple, where each person is age 18 or older and only has Supplemental Security Income (SSI) or SSI and Social Security (RSDI) income. These populations can use the form to renew Medical Assistance (MA) including MA payment of long-term care services, Minnesota Supplemental Assistance (MSA), Group Residential Housing (GRH), Supplemental Nutrition Assistance Program (SNAP) or a combination of these programs. Approximately 4,000 people per month are expected to use the form. Workers can check SIR announcements for more details.

County staff working on cost-effective insurance, third-party liability (TPL) or benefit recovery for clients in Minnesota Health Care Programs are encouraged to use this phone listing (PDF) to reach the correct DHS help desk or staff when calling with questions.
