DHS Program Resources Child and Teen Checkups Coordinators

Child and Teen Checkups (C&TC) Child and Teen Checkups (C&TC) logo

Child and Teen Checkups (C&TC) is Minnesota’s Early and Periodic Screening, Diagnostic and Treatment (EPSDT) program. EPSDT is a federal program required in every state to provide comprehensive preventive health care and dental services for children birth through 21 years who are enrolled in Medicaid.

Periodic examinations or screenings are delivered according to the C&TC Schedule of Age-Related Screening Standards and is key to ensuring participants receive appropriate preventive, dental, social-emotional, developmental and specialty services using the following methods:

Early: Assessing and identifying problems early

Periodic: Checking children's health at periodic, age-appropriate intervals

Screening: Providing physical, mental, developmental, dental, hearing, vision and other screening tests to detect potential problems

Diagnostic: Performing diagnostic tests to follow up when a risk is identified

Treatment: Control, correct or reduce health problems found

Minnesota’s Child and Teen Checkups (C&TC) Schedule of Age-Related Screening Standards (periodicity schedule) is based on the recommendations of the American Academy of Pediatrics (AAP) and the United States Preventive Services Task Force (USPSTF). The Minnesota Department of Human Services (DHS) works with the Minnesota Department of Health (MDH) to update the schedule according to federal requirements, state legislation and the unique needs and epidemiology of Minnesota's C&TC-eligible population following a four-year review cycle.

Oral health is a component of C&TC, and recommendations of care can be found on the Minnesota Schedule of Age-Related Dental Standards.

Coordinator Handbook

The Coordinator Handbook:

  • Outlines program requirements based on state and federal rules and regulations, the state Medicaid manual and DHS program policy
  • Offers program guidance to assist in planning and providing administrative services to eligible children and their families
  • Serves as an important tool for staff orientation and training

Brochures and other materials

DHS offers brochures and other materials through the DHS Bulk Order website and the searchable document library, eDocs.

CATCH 3

CATCH 3 is a software system used by Coordinators and outreach staff to track and document outreach and follow-up activities to enrolled children and families. The CATCH 3 User Manual is a guide to using the software, including how to install it on a computer.

imageCATCH 3 Letters

These letters are sent by county and tribal outreach workers to eligible children and their families. They introduce, inform and remind families of the benefits and notify them when the next medical screening is due, according to the periodicity schedule. Copies of these letters can be found on the C&TC SharePoint and the searchable document library, eDocs.

Providers will find many resources on the MHCP Partners and Providers webpages, including the Provider Manual, C&TC specific information, training opportunities and provider news and updates. Providers can subscribe to email lists and automatically receive updated information on C&TC and other topics.

Provider Manual

The Child and Teen Checkups section of the MHCP Provider Manual is the primary source for coverage policies, rates and billing procedures and is updated on an ongoing basis.

Documentation Forms

The DHS-4813 series of C&TC documentation forms can be used as guides or templates that include all screening components for each age-related visit following the periodicity schedule. Refer to the Minnesota C&TC Schedule of Age-Related Screening Standards for visit requirements and recommendations and to the C&TC Fact Sheets on each component for more information.

Reports

CMS-416 Report

The CMS-416 Report is a federal requirement for Minnesota’s annual reporting to the Centers for Medicare & Medicaid Services (CMS) on the EPSDT program. This report measures statewide participation rates and other data. County, tribal, and Integrated Health Partnership (IHP) specific reports are generated from the CMS-416 for a look at more local data.
