Case manager responsibilities

Background

The screening process is a case manager/service coordinator responsibility. Case management services for persons with developmental disabilities or related conditions (Rule 185) are a county social services agency responsibility as set forth in Minn. R. 9525.0004 to 9525.0036.

Familiarity with the content of Rule 185, other Minnesota rules governing services for persons with developmental disabilities or related conditions and relevant instructional and informational manuals and bulletins from the Department are necessary to use of the DD Screening Document and codebook.

Preparation

Before the screening team is convened:

1. Begin to enter information on the DD Screening Document.
2. Gather information before the meeting to help move the meeting along smoothly. For example, finding out at the meeting that a person needs a guardian stops the process and delays needed services/supports available to the person. DSD has prepared tools that can assist you in this process:

  • Convening the screening team

    Case managers/service coordinators must convene screening teams to evaluate level of care needed by people whenever people:

  • · Are at risk of placement in an intermediate care facility for persons with developmental disabilities (ICF/DD)
  • · Are at risk of nursing facility placement
  • · Request services in the areas of residential, day training and habilitation or family support.
  • Communication

    Case managers/service coordinators are responsible to:

    1. Notify members of the screening team, including the person's advocate, if any, of the screening date and time.

    2. Schedule screening meetings at a time and place that ensures the participation of all screening team members.

    3. Prior to completing the DD Screening Document, explain to people and their legal representatives, if any, the following:

  • · Person’s diagnosis
  • · Assessment information
  • · Screening/service planning process.
  • Remember to complete the DD Screening Document based on assessment information and not on team consensus.

    Timelines

    Case managers/service coordinators must:

    1. Convene screening teams within:

  • · Sixty (60) working days of when people or their legal representatives request services
  • · Five (5) working days of an emergency admission to an ICF/DD.
  • 2. Complete DD Screening Documents.

    3. Submit completed documents for approval through the Medicaid Management Information System within 90 days of the date of the screening meeting or MMIS will reject the screening document.

    4. If rejected, screening teams need to reconvene.

    DHS reviews DD Screening Documents within 10 working days of receiving the document.

    Commitment proceedings

    Use the screening process when people with a diagnosis of developmental disabilities are the subject of commitment proceedings. Minn. Stat. §253B.097

    Screening teams:

  • · Receive a referral
  • · Make recommendations to the court as needed
  • · Make recommendations and report available to the pre-petition screening unit according to the Data Practices Act, Minn. Stat. Chapter 13.
  • Correctional proceedings

    People under jurisdiction of a correctional agency

    Case managers/service coordinators must consult with corrections administrators on additional health, safety and supervision needs.
