Background

Annual review summarizes information by documenting:

  • · Continued eligibility for services and authorized level of care
  • · Informed choice of services among feasible alternatives
  • · Summary of the service plan that reflects an annual review and revision of the service plan as needed.
  • Timelines

    Conduct annual reviews within or at least every 366 days.

    Annual review requirements

    Complete annual reviews when:

  • · Full-team screenings cannot be completed within the required 366 days
  • · A person is at risk of placement in a nursing facility
  • · A person requests or receives services in the following areas: family support, nursing facility, residential, training and habilitation
  • · A person requested but does not yet receive as this updates the state waiting list
  • · Screening teams are not required to convene.
  • Eligibility span update

    Annual review documents update the eligibility span on recipient files in MMIS. Failure to submit annual review documents or other appropriate screening documents may result in loss of payment for services provided after the end of the eligibility span.

    MMIS

    The Disability Services Division does not automatically review screening documents submitted to MMIS other than full-team screenings. All screening documents submitted to MMIS are subject to MMIS system edits. For more information, refer to DD Screening Document Edits (PDF).
