Overview | When a comprehensive multi-disciplinary evaluation (CMDE) provider completes CMDE Medical Necessity Summary Information, DHS-7108 (PDF), they must use information from the person’s most recent medical evaluation. The person must have a medical evaluation completed within the last 12 months to meet the requirements of the initial CMDE or CMDE update. The person’s physician or advanced practice nurse must complete the medical evaluation (e.g., well child check or annual physical), and the CMDE provider must retrieve this information. | ||
Child and Teen Checkups | A medical evaluation is required as part of Medicaid – Early and Periodic Screening, Diagnostic and Treatment (EPSDT) guidelines for children younger than age 21 who are enrolled in Medicaid. EPSDT is a federal program required in every state to ensure children receive comprehensive care across health care providers. Child and Teen Checkups (C&TC) is Minnesota’s EPSDT program. DHS administers the program, and the Minnesota Department of Health (MDH) provides technical and clinical assistance. For more information, refer to DHS – C&TC. The C&TC program recommends children younger than age 21 receive periodic medical evaluations or screenings according to C&TC Schedule of Age-Related Screening Standards, DHS-3379 (PDF), also known as the periodicity schedule.
