Sources of income and acceptable verification documents

Program HH will not accept tax forms from previous years as verification of income unless the person is seasonally or self-employed. Program HH does not accept bank statements as verification for any source of income.

Gross wages and salary | A copy of all pay stubs received in the last 30 days or an employer-signed statement of gross monthly income or taxes (only if seasonally employed for a full year) |
Social Security Disability Insurance (SSDI) | Most recent award letter from Social Security Administration (SSA) |
Social Security Supplemental Income (SSI) | Most recent award letter from SSA |
Dividends and interest | Most recent Internal Revenue Service (IRS) Schedule B (Form 1040) or most recent Form 1099 |
Net rental income, estate/trust income and/or royalties | Most recent IRS Schedule E (Form 1040) |
Public assistance | Most recent award letter |
Pensions, annuities and/or veteran’s pensions | Most recent letter stating pension/annuity amount |
Unemployment, long-term and short-term disability insurance and/or worker’s compensation | Most recent letter stating amount of unemployment, long-term and short-term disability insurance and/or worker’s compensation benefit |
Self-employment | Most recently filed IRS Form 1040 tax return or business-income-to-expenses breakdown (if newly self-employed or if taxes have not yet been filed) |
