Assessor candidate education and experience

• County and tribal nations sample form: MnCAT certified assessor tracking form (PDF) — This is a sample form and may be used to collect and support an assessor candidate’s statutory education and experience requirements.
• The contract between DHS and a managed care organization (MCO) outline the requirements for the MCO and delegate agency. MCO delegates may refer questions to the MCO contract.

Note: All assessors who are certified in MnA before the launch of the MnCHOICES application will complete their MnCHOICES Content, MnCHOICES Assessment and MnCHOICES Access and Navigation modules as part of their recertification – see MnCAT Step 4: CLUs guidance for updates to MnCAT.
