- Technical: Report a problem with the MnCHOICES application, re-open a plan, recover data or report other computer-related issues.
- Policy: Assessments, support planning, waiver/Alternative Care (AC)/Essential Community Supports (ECS) programs, homecare [personal care assistance (PCA)/Community First Services and Supports (CFSS) or consumer directed community supports (CDCS)], rates, OBRA Level I [1] and II [2], managed care [health risk assessment (HRA), support plans or programs], LTSS (Long-term services and supports) Improvement Tool, MnCHOICES training or other.
- Other: User access, certification or other.
