MnCHOICES

MnCHOICES is Minnesota’s assessment and support planning system used by counties, tribal nations and managed care organizations. This web-based application allows lead agencies to do assessments and support planning in the same application for everyone who wants to use Minnesota’s long-term services and supports (LTSS).

Applicable legislation: Minnesota Statues, section 256B.0911.

News and updates

DHS published a report that reviewed state and lead agency processes related to MnCHOICES eligibility determination, assessment and support planning. DHS conducted this business process review to:

• Find ways lead agencies can make their workflows more efficient.
• Help increase the capacity of lead agencies to serve people and improve workloads for their staff members.
