Case Manager/Care Coordinator MHCP Information

Definitions

Case manager/care coordinator - Provider Type 23: Person employed by the local lead agency to provide case management services to persons.

Care coordinator - Provider Type 27: Lead agency staff contracted by health plans to provide care coordination activities for waiver recipients enrolled in managed care.

In some cases, this is in addition to their provider number type 23 as a county case manager. Only one care coordinator provider number is needed even if the care coordinator is contracted with more than one health plan.

Policy Statement

Each case manager/care coordinator must be enrolled as a Minnesota Health Care Programs (MHCP) provider and maintain an active provider enrollment status.

Assigning MHCP Provider Numbers

Each county is responsible to enroll and maintain case manager/care coordinator information for their employees.

County staff who have waiver/accounting security access are able to assign MHCP provider numbers to new case managers/care coordinators and update case manager/care coordinator information in the Medicaid Management Information System (MMIS) provider subsystem.

All care coordinator enrollment information is maintained in the MMIS provider subsystem.

Security Access

Contact your county MMIS liaison to locate county staff that have security access to add and update information in the MMIS Provider subsystem.

Assigning a MHCP Provider Number To A New Case Manager/Care Coordinator

Instructions for the LTC Screening Document and Service Agreement: refer to the section titled Case Manager Provider Number.

NOTE: When enrolling a case manager/care coordinator as a Minnesota Health Care Programs (MHCP) provider a Social Security number is required.

Updating Case Manager/Care Coordinator information

It is important to keep all information about a case manager/care coordinator current in the provider enrollment file within the MMIS provider subsystem. If the case manager/care coordinator’s name, address, facsimile or phone number changes, update MMIS to reflect any of those changes.

On the MMIS Main Menu screen.
Select the Provider File Application and transmit.
On the PKEY screen - Provider subsystem

1. Enter a "C" in the action code field.

2. Make the appropriate changes.

3. Verify the enrollment status remains "3.”

4. Hit PF3 to save the changes.

Call any changes needed to the information on the provider record for a care coordinator, Provider Type 27, into the DHS Provider Enrollment Unit at (651) 431-2700 or 1 (800) 366-5411. Select options for provider enrollment.

NOTE: When updating a case manager/care coordinator record in the MMIS provider subsystem a Social Security number is required.

Additional Resources

Notify the Provider Enrollment Unit in writing of any changes to the information listed in the MMIS provider subsystem.
