Definition

Short-term admission to an ICF/DD from the DD Waiver: When a person who receives Developmental Disabilities (DD) Waiver supports and services, including consumer-directed community supports, chooses to disenroll from the DD Waiver for short-term admission to an intermediate care facility for persons with developmental disabilities (ICF/DD).

Eligibility

A person must meet admission and eligibility criteria for ICF/DD, regardless of the anticipated length of stay in the ICF/DD.

DD Waiver allocation

A lead agency is required to maintain a person’s waiver allocation when a person is discharged from the DD Waiver for short-term admission to an ICF/DD. The lead agency informs the person in writing, prior to the waiver discharge that the:

  • · DD Waiver allocation will be held for the person
  • · Person may be re-admitted to the DD Waiver upon discharge from the ICF/DD.
  • When informing the person in writing, the lead agency may use one letter for one year, regardless of how many times the person is discharged from the DD Waiver during that year. The letter should include the date span covered by the letter.

    Discharge from ICF/DD

    If upon discharge from the short-term ICF/DD stay, a person is no longer eligible for the DD Waiver allocation being held, the lead agency must:

  • · Develop an alternative discharge plan that meets the person’s needs
  • · Provide the person with fair hearing rights (i.e., appeals) information (see CBSM – Appeals).
  • CDCS

    Consumer directed community supports (CDCS) is one service available under the DD Waiver plan. If a person receives CDCS and disenrolls from the DD Waiver more than once during his/her CDCS service plan year, the person is ineligible for CDCS for the remainder of that CDCS service plan year.     If a person stops CDCS, other services are available through the DD Waiver that may meet the person’s needs. For more information, see CBSM – DD Wavier.

    ICF/DD reimbursement

    A person admitted to an ICF/DD facility will be charged the same daily rate as any other ICF/DD admission covered by the state plan, as is the current practice. The daily rate is the same for an admission, regardless of the anticipated length of stay. For more information, see ICF/DD rates.
