Definition

CDCS community support plan: A plan developed by a person who chooses to self-direct their services through CDCS. The person’s lead agency must review and approve the person’s CDCS community support plan.

Overview

A person who chooses to self-direct their services through CDCS is responsible to develop an annual CDCS community support plan by following the process described on CDCS Manual – CDCS process and procedure.

Format

To write their community support plan, the person can use:

  • · CDCS Community Support Plan, DHS-5788A.
  • · An alternate format with all required plan components.
  • DHS recommends using CDCS Community Support Plan, DHS-5788A because it includes all required components and performs budget calculations.

    If a person chooses to use an alternate format, they must:

  • · Include all required plan components.
  • · Print and sign the participation agreement section of CDCS Community Support Plan, DHS-5788A.
  • Required plan components

    The CDCS community support plan must describe:

  • · Person’s assessed needs.
  • · Goods and services the person will use to meet their assessed needs.
  • · Informal supports the person will use to meet their assessed needs.
  • · Safeguards to reasonably maintain the person’s health and safety.
  • · Person’s emergency needs and how they will meet them.
  • · Plan for monitoring and evaluating the effectiveness of the services.
  • · Person’s budget.
  • · Expected service outcomes.
  • · Individual(s) responsible for monitoring the plan, in addition to the lead agency and the person.
  • · Qualifications and training needs of directly hired workers.
  • · Required qualifications (i.e., certification, licensing and/or waiver enrollment requirements) and training for each provider based on the service provided (and additional qualifications, if the person desires).
  • Service categories

    CDCS Community Support Plan, DHS-5788A contains sections for all eight service categories. The person only completes the sections for the categories they intend to use.

    Note: All people must use the financial management services (FMS) service category, so the FMS section is required.

    Medical Assistance (MA) state plan services

    If the person is using one or more of the following MA services, they must include them in their CDCS community support plan and CDCS budget:

  • · Personal care assistance (PCA).
  • · Community First Services and Supports (CFSS).
  • · Home care nursing.
  • · Skilled nurse visits.
  • If a person is using CFSS, they must also complete CFSS Service Delivery Plan, DHS-6893P.
