Profile Report

Overview

Use this page to review and print your profile report.

Your profile report is a summary of your profile with MHCP. You can print and file a copy of your profile report for your records. Your profile report will be updated after Provider Screening and Enrollment reviews and approves every request you submit. The request will show the status of complete.

The information in your profile report reflects the information of your master profile with MHCP. Your master profile with MHCP is your information that is confirmed and used in our system. The sections within your profile report appear only when there is information in that section within your master profile. For example, if you do not have any services within your enrollment records, the services section will not appear.

Your profile report information is at the top of your profile report, then your enrollment record(s) information. If you have more than one enrollment record, your profile report information on your enrollment records will be grouped by enrollment record. After your enrollment record information is your owner or authorized person or business information.

Organization Information

The Organization Information section displays the information on your Manage Organization Information page.

Individual Information

The Individual Information section displays the information on your Manage Individual Information page.

Profile Notes

The Profile Notes section displays information on your Profile Notes page.

Enrollment Record Information

The Enrollment Record Information section displays the information on your Enrollment Record Information page.

Physical Address

The Physical Practice Address section displays the information on your Manage Physical Address page.

Mailing Addresses

The Mailing Addresses section displays information on your Manage Mailing Address page. If you have more than one mailing address, information appears for both of your mailing addresses.

File Location Address

The File Location Address section displays information on your Manage File Location Address page.

Provider Identifiers

The Provider Identifiers section displays information on your Manage Provider Identifier page. If you have more than one provider identifier, information appears for all of your provider identifiers.

Services

The Service section displays information for all of your services on your Manage Service page.

Service Related Information

The Service Related Information section displays information on your Manage Service Related Information page.

Additional Enrollment Questions

The Additional Enrollment Questions section displays information on your Manage Additional Enrollment Questions page.

Credentials

The Credentials section displays information for all of your credentials on your Manage Credential page.

Fee Information

The Fee Information section displays information for all of your fees on your Manage Fee page.

Payment Details

The Payment Details section displays information for your current fee payment on your Fee Payment Details page.

Site Visits

The Site Visits section displays information on your Site Visit page. If you have had more than one site visit, information for each of your site visits appears.

Facility/Agency Identifiers

The Facility/Agency Identifiers section displays information for all of your facility or agency identifiers on your Manage Facility Agency Identifier page.

Agreements and Addendums

The Agreement/Addendum section displays information for all of your agreements and addendums on your Manage Agreement and Addendum page.

Direct Support Worker Training Information

The Direct Support Worker Training Information section displays information on your DSW Training Information page. If you have enrolled in or completed more than one training course, information for each training appears.

Direct Support Worker Background Study Information

The Direct Support Worker Background Study Information section displays information on your Manage DSW Background Study Information page.

Limiting Caseload

The Limiting Caseload section displays information on your Manage Limiting Caseload page.

Enrollment Record Notes

The Enrollment Record Notes section displays information on your Manage Note page.

Enrollment Status

The Enrollment Status section displays your current enrollment record status.

Organization to Individual Provider Affiliation

The Organization to Individual Provider Affiliation section displays information on your Manage Organization to Individual Affiliation page. Information appears for each individual provider affiliated with your organization.

Organization to Direct Support Worker Affiliation

The Organization to Direct Support Worker Affiliation section displays information on your Manage Organization to Direct Support Worker Affiliation page. Information appears for each direct support worker affiliated with your organization.

Trading Partner to Service Provider Affiliation

The Trading Partner to Service Provider Affiliation section displays information on your Manage Trading Partner to Service Provider Affiliation page. Information appears for each service provider affiliated with your trading partner organization.

Individual to Organization Affiliation

The Individual to Organization Affiliation section displays information on your Manage Individual to Organization Affiliation page. Information appears for each organization you are affiliated with as an individual provider.

Direct Support Worker to Organization Affiliation

The Direct Support Worker to Organization Affiliation section displays information on your Manage Direct Support Worker to Organization Affiliation page. Information appears for each organization you are affiliated with as a direct support worker.

Service Provider to Trading Partner Affiliation

The Service Provider to Trading Partner Affiliation section displays information on your Manage Service Provider to Trading Partner Affiliation page. Information appears for each trading partner affiliated with your organization.

Payment Withholds

The Payment Withhold section displays information on Manage Payment Withhold page. If you have more than one payment withhold, information for each payment withhold appears.

Owner/Authorized Persons

The Owner/Authorized Person section displays information on your Manage Owner/Authorized Person Detail page for all of your owners and authorized persons.

Owner/Authorized Businesses

The Owner/Authorized Business section displays information on your Manage Owner/Authorized Business page for all of your owners and authorized businesses.

Owner/Authorized Person Role
