Summary Report

Overview

Your summary report shows the documentation you uploaded with your request, enrollment record or profile. The uploaded documentation that appears within your summary report is dependent on what page you access your summary report. Views can be limited by the role that has been assigned to you in the system.

You can access your summary report from the Manage Portfolio page for new providers or the Manage Portfolio page for existing providers from the Master Profile section or the Requests section of the page. From that page, you can review all of the uploaded documentation within your entire profile. The sections on this page that appear for your summary report are grouped by enrollment record.

When accessing your summary report from the Manage Enrollment Records page, you can review only the uploaded documentation within that specific enrollment record.

Regardless of what page you access your summary report from, only the sections that have uploaded documentation will appear.

Use this page to view your summary report, access your profile report and download all of the documents within your summary report for your records.

The Enrollment Record Information section on this page will appear only when you review your summary report for a specific enrollment record.

Provider Profile

Under Provider Profile, find a link to view your profile report.

Click View to go to and view your profile report.

Profile Identifier

Under Profile Identifier, find the document(s) you uploaded to Supporting Documentation on the Manage Profile Identifier page.

Supporting Documentation

The Supporting Documentation box shows your uploaded document.

Personal Care Assistance (PCA) Personnel Information

Under PCA Personnel Information, find the document(s) you uploaded to PCA Agency Personnel List on the Enrollment Record Information page.

Tribe Selection

Under Tribe Selection, find the document(s) you uploaded to Tribal Contract Documentation on the Manage Tribe page.

Under Service Provider to Trading Partner Affiliation, find the document(s) you uploaded to File Uploaded on the Manage Service Provider to Trading Partner Affiliation page.

Owner/Authorized Person Training Course Information

Under Owner/Authorized Person Training Course Information, find the document(s) you uploaded to Certificate Documentation on the Manage Owner Authorized Person Training Course page.

Certificate Documentation

The Certificate Documentation box shows your uploaded document.

Click View to view your uploaded document.

Download All Documents

Click Download All Documents to download your profile report and all of your attachments.

After you click Download All Documents, the system will create a zip file that contains your profile report and all of your attachments, and prompt you to save the file to your computer.
