Manage Organization Information

Overview
On this page, you can view or modify your organization’s profile information when you are creating a new profile or manage your existing organization profile information. Information on this page can only be modified when a user is making a Global Request.
You will use this page to report a sale or transfer of ownership, assets, or control of your organization. Providers reporting a sale or transfer of ownership, assets, or control of organization are encouraged to call the Provider Resource Center at 651-431-2700 or 800-366-5411 and request assistance from Provider Eligibility and Compliance as requests to change ownership can be complicated.

Organization Information
Under the Organization Information section, provide information related to your organization.
Legal Name
Type your organization’s legal name according to the IRS in the Legal Name box.
Fiscal Year End
Type the day and month that your organization’s fiscal year ends in the format of MM/DD in the Fiscal Year End box. The default fiscal year end date is 12/31 and auto populates in this box.
Ownership Type
Select your organization’s ownership type from the Ownership Type drop-down list.
The options you can select from are:
Ownership Type Other Description
Type your ownership description in the Ownership Type Other Description box if your selected ownership type is Other.
This box will only appear if you selected Other as your ownership type in the Ownership Type box.
Email Address
Type a valid email address in the Email Address box.
This box is optional. You do not have to give an email address, but if you do, you must provide a valid email address. A valid email follows this format: xxx@yyy.com. For example, JohnDoe@organization.com is a valid email address. JohnDoeOrganization.com and JohnDoe@organization are not valid because the first is missing the “@ sign” and the second is missing “.com”. Other versions of .com are also acceptable such as .edu, .org, .gov., and others.
Supporting Documentation
The Supporting Documentation box shows the documentation that you uploaded to support your request to change your organization information.
Remove Upload
Check the Remove Upload box to remove the document that is currently uploaded. This will happen automatically if there is an existing uploaded document and you upload a new document.
Upload Supporting Documentation
Use the Upload Supporting Documentation box to upload documentation you have to support your request to change your profile identifier.
This option only appears when the user is making a Global Request. This field is only required if a provider is changing their organization information after their initial enrollment has been approved.
Click on the words Upload Supporting Documentation or click on the icon shaped like an eye to open a dialogue box on your computer which will allow you to select a document to upload. MPSE only allows the following document types for upload: PDF, JPG, and PNG.
Reason for Change Note Actions
Use Reasons for Change Note Actions to manage notes about your organization information.
Click Edit to add a new note or edit an existing note about your request to change your organization information. This option only appears when the user is making a Global Request.
Click View to review the note about changes made to your organization information. In view mode, you cannot make edits.
Continue
Click Continue to save your changes and go to the next page.
Cancel
Click Cancel to go back to the previous page.
