Overview
On this page, you will view or modify your answers to the additional enrollment questions for your enrollment record. You are required to answer all additional enrollment question before you can continue with the enrollment process. You can only edit the information on this page if you are in an MPSE Request.
Managed Care Organizations (MCOs) use the answers on this page for reporting purposes and the answers do not impact the final enrollment decision Minnesota Health Care Programs (MHCP) makes

Additional Enrollment Questions
Under Additional Enrollment Questions, provide your answers to the additional enrollment questions.
Do you provide 24-hour emergency coverage?
Select the Yes option if you provide 24-hour emergency coverage. Select the No option if you do not provide 24-hour emergency coverage.
This does not mean that the clinic must be open for regular business 24 hours per day. For example, a clinic that may have set office hours but has an emergency contact number that members can call in emergencies would answer Yes to this question.
Do you provide same-day urgent care?
Select the Yes option if you provide same-day urgent care. Select the No option if you do not provide same-day urgent care.
See Minnesota Rules, 4654.0200 for the definition of urgent care.
How far are you from public transportation?
Type the distance between your practice facility and the nearest public transportation in the How far are you from public transportation? box.
Use the imperial system of measurement when entering data; example: miles, feet, inches.
What are your hours of operations?
Type your normal business hours in the What are your hours of operations? box. For example, if your normal business hours are Monday through Friday 8:00 a.m. to 8:00 p.m., type M-F 8AM-8PM.
Do you offer flexible appointment hours?
Select the Yes option if you offer flexible appointment hours. Select the No option if you do not offer flexible appointment hours.
Do you offer non-English and American Sign Language interpreter services?
Select the Yes option if you offer non-English and American Sign Language interpreter services. Select the No option if you do not offer non-English and American Sign Language interpreter services.
Do you offer language-line interpreters?
Select the Yes option if you offer language-line interpreters. Select the No option if you do not offer language-line interpreters.
“Language-line interpreters” refers to interpreting services that can be provided directly over the phone without needing to pre-arrange the interpreting services.
Do you have adequate seating in reception areas?
Select the Yes option if you have adequate seating in your reception areas. Select the No option if you do not have adequate seating in your reception areas.
Is the building or facility easily identified and accessible to people with disabilities?
Select the Yes option if your building or facility is easily identified and accessible to people with disabilities. Select the No option if your building or facility is not easily identified and accessible to people with disabilities.
Does the parking lot provide parking ramp or parking lot accessibility to people with disabilities?
Select the Yes option if your parking lot provides a parking ramp or parking lot accessibility to people with disabilities. Select the No option if your parking lot does not provide a parking ramp or parking lot accessibility to people with disabilities.
Are the handicapped parking spots wide enough to accommodate side lift systems in vehicles?
Select the Yes option if your handicapped parking spots are wide enough to accommodate side lift systems in vehicles. Select the No option if your handicapped parking spots are not wide enough to accommodate side lift systems in vehicles.
Are patient care areas accessible to people with disabilities?
Select the Yes option if your patient care areas are accessible to people with disabilities. Select the No option if your patient care areas are not accessible to people with disabilities.
What specific accommodations do you have available for people with physical disabilities?
Type any specific accommodations that are available to people with physical disabilities at your practice in the What specific accommodations do you have available for people with physical disabilities? box.
Specific accommodations include, but are not limited to, elevators, single-floor buildings, ramps, wide hallways, and stair lifts.
Do you currently have a contract with any of the following health plans through a Prepaid Medical Assistance Program (PMAP)?
Select the Yes option if you currently have a contract with a health plan through a PMAP. Select the No option if you do not currently have a contract with a health plan through a PMAP.
If you select Yes to this question, a list of PMAP health plans will be displayed. The current health plans available for selection are:
To identify which PMAP(s) you contract with, click the name of the health plan in the Available PMAP list. This will move the selection from the Available PMAP box to the Selected PMAP box. You can remove any selected PMAP options by clicking the name of the health plan in the Selected PMAP box to return it to the Available PMAP box.

Click Continue to save your changes and go to the next page.

Click Cancel to go back to the previous page.
