Manage Notes

Overview
Use the notes page within your profile for communication between you and Minnesota Health Care Programs (MHCP). There are two pages in the Minnesota Provider Screening and Enrollment (MPSE) portal where notes can be viewed or added, Profile Notes and Notes. You can only add or edit notes in the Notes page when you are in an MPSE Request.
The Profile Notes page is used for notes regarding the entire MPSE portfolio, including the owners and authorized person sections. The Notes page is specific to the enrollment record it appears in and is used for notes specific to that enrollment record, including Request for More Information letters.
The notes that MPSE displays are determined by your User Role. If you have a global user role, all notes will be displayed. If you have an affiliations user role, only notes made by others with an affiliations user role will be displayed.

Enrollment Record Information
This is a recurring section that appears on multiple MPSE pages for informational purposes only. See Enrollment Record Information on the Recurring Items page of the MPSE User Manual. This section does not display on the Profile Notes page.
Manage Notes
The Manage Notes section displays information on the notes that exist on this page. If there are no notes to display, you will see the following message: “There are no items in the list to display.”
More
The Note text column will display a shortened version of the full note. Click More to expand the column and display the full note.
Less
Click Less to change the display of the Note text column to the shortened version of the note. This option is only available if More was selected in the Note text column.
View/Edit
Click View/Edit to go to the Manage Note page. From this page, you will be able to review and edit the note as needed.
Delete
Click Delete to delete the note from your profile. You can only delete a note if you added the note during this MPSE request. Notes can only be deleted by the user that added the note.
View
Click View to go to the Manage Note page. From this page, you can review the full note text and access any documents uploaded with the note.
Add a Note
Click Add a Note to go to the Manage Note page.
Continue
Click Continue to go to the next page.

This is a recurring section that appears on multiple MPSE pages for informational purposes only. See Enrollment Record Information on the Recurring Items page of the MPSE User Manual. This section does not display on the Profile Notes page.
Manage Note
In the Manage Note section, type your note and use the Upload Note Documentation field to upload supporting documentation, if needed.
Note Text
Type your note in the Note Text box. There is a limit of 4000 characters per note.
Supporting Note Documentation
The Supporting Note Documentation field displays uploaded documentation if you have uploaded any documents. Click the page icon to open the document in a new tab.
Remove Upload
Check the Remove Upload box to remove the document that is currently uploaded. This will happen automatically if there is an existing document, and you upload a new document. This box will not appear if there is no document uploaded in the Supporting Note Documentation field.
Upload Note Documentation
Use the Upload Note Documentation field to upload any supporting documentation you want to add to support your note. Supporting documentation is not required. Click on the words Upload Note Documentation or click on the icon shaped like an eye to open a dialogue box on your computer which will allow you to select a document to upload. MPSE only allows the following document types for upload: PDF, JPG, and PNG.
User Name
The User Name field displays the user name of the person that created the note. If Provider Eligibility and Compliance created the note, the field will display Provider Enrollment.
Update Date
The Update Date field displays the date the note was created or last updated.
Continue
Click Continue to save your changes and go to the next page.
Cancel
Click Cancel to go back to the previous page.
