  • Overview

    Each business you add as an owner or authorized business with your organization must have at least one role type assigned. Use this page to add, view, or edit the role types for businesses you add. You can only edit the information on this page if you are in an MPSE Request.

    Minnesota Health Care Programs (MHCP) requires that all businesses with a direct or indirect ownership or controlling interest of five percent or more are disclosed. Businesses considered board members or officers of this organization must also be disclosed.

    Portfolio Information

    This is a recurring section that appears on multiple Minnesota Provider Screening and Enrollment (MPSE) pages for informational purposes only. See Portfolio/Profile Information on the Recurring Items page of the MPSE User Manual.

    Owner/Authorized Business Legal Name

    The Owner/Authorized Business Legal Name section displays the legal name of the owner or authorized business whose role types you are currently managing.

    Owners/Authorized Business Roles

    The Owner/Authorized Business Roles section displays all of the role types that have been added for the owner or authorized business. If the owner or authorized business does not yet have any assigned role types, this section will not have any information to display.

    View/Edit
    Click View/Edit to go to the Manage Owner/Authorized Business Role page. From this page, you are able to review and edit the information for that role type.

    Delete
    Click Delete to delete the role type.

    View
    Click View to go to the Manage Owner/Authorized Business Role page. From this page, you can view the information for that role type. You will not be able to make any changes while in View mode.

    Add a Role Type

    Click Add a Role Type to go to the Select Owner/Authorized Business Role page.

    Click Continue to go to the next page.

    Select Owner/Authorized Business Role

    Overview

    On this page, you can select the role type for an owner or authorized business.

    Owner/Authorized Business Legal Name

    The Owner/Authorized Business Legal Name section displays the name of the owner or authorized business whose role types you are currently managing.

    Owner/Authorized Business Role Type

    In the Owner/Authorized Business Role Type section, select a role type for the owner or authorized business.

    Role Type

    Select a role type from the Role Type drop-down list.

    The available role types to select from are:

  • · Board Member or Officer: Any business that is a board member or officer of the enrolling organization.
  • · Managing Employee: A business responsible for operational or managerial control over, or directly or indirectly conducts or manages the day-to-day operations of an institution, organization, agency or school.
  • · Other: For a disclosed business that does not fit into any of the other role types.
  • · Owner/Managing Interest: Any business or organization with direct or indirect ownership of five percent or more in the disclosing entity.
  • · Subcontractor: Subcontractor means an agency or organization to which a disclosing entity has contracted with or delegated some of its management functions or responsibilities of providing medical care to its patients.

    Manage Owner/Authorized Business Role

    Owner/Authorized Business Legal Name and Role Type

    The Owner/Authorized Business Legal Name and Role Type section displays the legal name and role type of the owner or authorized business whose role types you are currently managing.

    You are not able to edit this section from this page. If there is an error, return to the Select Owner/Authorized Business Role page and correct the information before continuing.

    Legal Name

    The Legal Name box shows the legal name of the owner or authorized business.

    Role Type

    The Role Type box shows the role type for the owner or authorized business you are currently managing.

    Owner/Authorized Business Role Information

    In the Owner/Authorized Business Role Information section, provide information on the role type that is shown in the Role Type box for the owner or authorized business whose legal name is shown in the Legal Name field in the Owner/Authorized Business Legal Name and Role Type section. If the information in the fields do not clarify your business structure, upload a note with additional information.

    The information you are required to provide in this section is determined by the role type you have selected. Fields that are not applicable to your role type will not appear or will not be editable.

    Other Role Description

    Type a description of the role type in 50 characters or less in the Other Role Description field if your selected role type is Other.

    Start Date

    Enter the date that the owner or authorized business started in this role in the Start Date field. The date must be in MM/DD/YYYY format.

    End Date

    Enter the end date for this role in the End Date field, if applicable. The date must be in MM/DD/YYYY format.

    Ownership Managing Control Type

    Select the ownership or managing control type from the Ownership Managing Control Type drop-down list.

    The options you have to select from are:

  • · 5% or more direct ownership
  • · 5% or more indirect ownership
  • · 5% or more mortgage ownership
  • · 5% or more security interest
  • Percent Ownership/Managing Interest

    Type in the numeric percentage of ownership in the Percent Ownership/Managing Interest field.

    The percent of ownership must be greater than or equal to five percent, and less than or equal to 100 percent. The total percent of direct ownership for all the direct ownership roles of a profile must not exceed 100 percent for all persons and businesses that are current owners.

    The total percentage of managing interest or indirect ownership may be greater than 100 percent.

    Enrollment Record

    Select the enrollment record associated to this owner or authorized business role from the Enrollment Record drop-down list.

    The drop-down list will contain all of the enrollment records that exist within your profile and are sorted by practice name and enrollment type.

    Cancel

    Click Cancel to go back to the previous page.
