Affiliation Request

Overview

Affiliations refer to a partnership between an organization and an individual provider or a service provider with a trading partner. There are five different types of affiliation requests within the Minnesota Provider Screening and Enrollment (MPSE) portal. The types of affiliation requests that a user can make depends on the portfolio type (either individual or organization) and the enrollment record type (such as nurse practitioner, hospital or home health agency). A user can only add or modify one affiliation per request. There is no option to add or modify multiple affiliations in one request. The different affiliation request types are:

· Individual to Organization Affiliation

Individual providers can request to affiliate with an organization by submitting an individual to organization affiliation request. Find the instructions to make this type of request on the Individual to Organization Affiliation section of this manual.

· Organization to Individual Affiliation

Provider organizations with enrollment record types that allow affiliations to individual health care providers can submit an organization to individual affiliation request. Find the instructions to make this type of request on the Organization to Individual Affiliation section of this manual. Please note that only portfolios with enrollment records allowing affiliations will see the link to manage organization to individual affiliations on the Manage Portfolio page of the MPSE portal.

· Organization to Direct Support Worker Affiliation

Provider organizations with enrollment record types that allow affiliations to direct support workers can submit an organization to direct support worker affiliation request. Find the instructions to make this type of request on the Direct Support Worker Affiliation section of this manual.

· Service Provider to Trading Partner Affiliation

A service provider can request to affiliate to a trading partner (such as a billing intermediary, clearinghouse, or EDI trading partner) by submitting a service provider to trading partner affiliation request. · Trading Partner to Service Provider Affiliation

A trading partner can request to affiliate to a service provider by submitting a trading partner to service provider affiliation request. Find the instructions to make this type of request on the Trading Partner to Service Provider Affiliation section of this manual.
