Manage Owner/Authorized Person Training Courses

    Minnesota Health Care Programs (MHCP) requires individuals listed as an owner or authorized person to an organization with an enrollment record of Home and Community-Based Services (HCBS) Waiver and Alternative Care (AC), Personal Care Assistant (PCA), and Community First Services and Supports (CFSS) to upload training certificates to this page.

    Use this page to manage any training courses required for the owner or authorized person named on this page. You can only edit the information on this page if you are in an MPSE Request.

    Owner/Authorized Person Name

    The Owner/Authorized Person Name section displays the name of the owner or authorized person whose training courses you are managing.

    Owners/Authorized Person Training Courses

    The Owners/Authorized Person Training Courses section displays all of the training courses that have been scheduled for or completed by the owner or authorized person named on this page. If the owner or authorized person has not yet had any training courses added, this section will not have any information to display.

    View/Edit
    Click View/Edit to go to the Manage Owner/Authorized Person Training Course page. From this page, you can review and edit the information related to this training course.

    Delete
    Click Delete to delete the training course. You can only deleted a training course when it was added during the current MPSE request.

    View
    Click View to go to the Manage Owner/Authorized Person Training Course page. From this page, you can view the information for the training course. You will not be able to make any changes while in view mode.

    Add a Course

    Click Add a Course to go to the Select Owner/Authorized Person Course Name page.

    Click Continue to go to the next page.

    Select Owner/Authorized Person Course Name

    Overview

    On this page, you can select the training course that an owner or authorized person has completed or is scheduled to complete.

    Owner/Authorized Person Name

    The Owner/Authorized Person Name section displays the name of the owner or authorized person whose training course you are managing.

    Owner/Authorized Person Course Name

    In the Owner/Authorized Person Course Name section, select the course that the owner or authorized person has completed or is scheduled to complete.

    Course Name

    Select a course from the Course Name drop-down list.

    The available courses to select from are:

  • · Steps for Success 1 Day
  • · Steps for Success 3 Day
  • · Waiver 101
  • · Waiver and AC Resources and MN–ITS Training
  • Continue

    Click Continue to save your changes and go to the next page.

    Click Cancel to go back to the previous page.

    Manage Owner/Authorized Person Training Course

    Owner/Authorized Person Name and Course Name

    The Owner/Authorized Person Name and Course Name section displays the name of the owner or authorized person and the name of the course that they have completed or are scheduled to take.

    Owner/Authorized Person Training Course Information

    In the Owner/Authorized Person Training Course Information section, provide information on the training course for the owner or authorized person shown under Owner/Authorized Person Name and Course Name.

    Find training requirements for PCA and CFSS providers on the PCA/CFSS Provider Agency Training Requirements page.

    Date Training Scheduled

    Enter the date that the owner or authorized person is scheduled to take the selected training course in the Date Training Scheduled Field. If the training was already completed, enter the date the owner or authorized person attended the selected training course.

    The date that the training is scheduled cannot be more than 180 days in the future.

    Date Training Completed

    Enter the date that the owner or authorized person completed the training course in the Date Training Completed field.

    This is only required if the course name is Waiver 101, Steps for Success 1 Day or Steps for Success 3 Day. For all other courses, it is optional.

    HCBS Waiver and AC providers must complete Waiver 101 before enrolling with MHCP. See the HCBS Waiver and AC Provider Training 101 page for information on how to complete this training.

    PCA and CFSS providers must complete Steps for Success training before enrolling with MHCP. See the PCA/CFSS Steps for Success page for information on how to complete this training, including who needs to take Steps for Success 1 Day versus Steps for Success 3 Day.

    Certificate Documentation

    The Certificate Documentation field shows your uploaded training certificate. Click the page icon to open the document in a new tab.

    Remove Upload
    Check the Remove Upload box to remove the document that is currently uploaded. This will happen automatically if there is an existing document and you upload a new document. This box will not appear if there is no document uploaded in the Certificate Documentation field.

    Upload Certificate Documentation

    Use the Upload Certificate Documentation box to upload a copy of the certificate demonstrating that the owner or authorized person has completed the selected training course. Click on the words Upload Certificate Documentation or click on the icon shaped like an eye to open a dialogue box on your computer which will allow you to select a document to upload. MPSE only allows the following document types for upload: PDF, JPG, and PNG.

    Click Continue to save your changes and go to the next page.

    Cancel

    Click Cancel to go back to the previous page.
