Transition from PCA to CFSS

DHS is in the process of replacing personal care assistance (PCA) and the Consumer Support Grant (CSG) with Community First Services and Supports (CFSS). For more information about this transition, refer to CFSS Manual – Transition from PCA and CSG to CFSS.

Similarities

The AHC3 screen is the same for a PCA and CFSS service agreement (SA).

Differences

There are no differences between a PCA and CFSS SA on the AHC3 screen.

Overview

This page describes how to complete the AHC3 screen for both PCA and CFSS.

The AHC3 screen contains comments about a PCA/CFSS SA that the county/tribal nation needs to communicate to DHS. The county/tribal nation and DHS also use the AHC3 screen for other services or programs. The county/tribal nation does not always use the AHC3 screen.

Comments the county/tribal nation enters on the AHC3 screen do not appear in the person’s SA letter (i.e., recipient letter) or provider’s SA letter.

System constraints

MMIS includes the following system constraints when the county/tribal nation enters comments:

  • · MMIS limits comments to a maximum of 12 lines of text. If the county/tribal nation needs more space, they can enter comments on the ADHS screen. The county/tribal nation presses the tab key to move to the next line.
  • · MMIS does not wrap text.
  • · MMIS does not check spelling.
  • Additional comments field (ADDITIONAL COMMENTS)

    In the ADDITIONAL COMMENTS field, the county/tribal nation enters additional information about this assessment or SA for DHS use. The county/tribal nation dates and initials all comments.

    The county/tribal nation uses the AHC3 screen to document:

  • · Reasons the SA assessment and/or MMIS entry is late.
  • · Name of the person’s second representative if the person has two or more responsible parties/participant’s representatives whose names do not fit on the ASA2 screen.
  • The county/tribal nation can choose to use the AHC3 screen to document:

  • · The units (CFSS agency model) or dollars (CFSS budget model) the person plans to use and/or the county/tribal nation. recommends if the person has the home care rating EN and will also use home care nursing.
  • · Additional information DHS needs to know.
  • Navigation

    After the county/tribal nation enters comments, they press a transmit key to advance to the ADHS screen.
