MMIS SERVICE AGMT – AHC2 screen for CSG

Transition from CSG to CFSS

DHS is in the process of replacing personal care assistance (PCA) and the Consumer Support Grant (CSG) with Community First Services and Supports (CFSS). For more information about this transition, refer to CFSS Manual – Transition from PCA and CSG to CFSS.

DHS must update the Medicaid Management Information System (MMIS) to allow counties/tribal nations to enter a CFSS service agreement (SA). Some fields have minor changes. For more information, refer to DSD MMIS Reference Guide – MMIS transition from PCA and CSG to CFSS.

During the transition from CSG to CFSS, the county/tribal nation follows the instructions below to enter a new SA in MMIS:

  • · Conducts the assessment.
  • · Enters three months of CSG services with the CSG procedure code (T2025).
  • · Enters a line for the county/tribal nation administrative fee with the CSG procedure code (T2025).
  • · Enters a line for consultation services. For more information, refer to DSD MMIS Reference Guide – ASA3 screen for consultation services (T1023).
  • PCA assessment results

    Enter the assessment results from the PCA Assessment and Service Plan (DHS-3244) in the equivalent assessment fields on the AHC2 screen. When cashing out a current service agreement for PCA enter the PCA assessment on the current PCA SA on the new SA for CSG.

    CD field

    Enter X in CD (code) fields to indicate needed activity. Leave CD fields blank when activity does not apply.

    FSG field

    Effective 08/01/2012 only N (no) is valid in FSG field when there is a line for T2025 UD (CSG with FSG) on the SA. Exception code 886 posts when field value is Y (yes) or blank and there is a line for T2025 UD (CSG with FSG) after Aug. 1, 2012. When the SA is for T2025 (CSG) only, the FSG field valid values must be N (no).

    REF COMP field

    Enter Y (yes) to indicate that the assessor made referral(s) for other services.

    Enter N/A (not applicable) when no referral for other services is required.

    EN field

    Enter an X in the EN field if the recipient is on a ventilator a minimum of 6 hours per day for a minimum of 30 days. Leave EN field blank when EN does not apply.

    PROV OWN/CTRL field

    Enter Y (yes), N (no) or U (uncertain) in the PROV OWN/CTRL field.

  • · Y (yes) in the PROV OWN/CTRL field indicates the PCA provider is owner or controls the housing arrangement.
  • · N (no) indicates the PCA provider is not owner.
  • · U (uncertain) indicates PHN is unable to determine if PCA provider is owner or controls the housing arrangement.
  • The SA end date cannot extend beyond Aug. 10, 2010 when the indicator in the PROV OWN/CTRL field on the AHC2 screen is Y (yes) or U (uncertain). MMIS posts exception code 401 (PROV OWN/CTRL FIELD-SHORTEN SA) when PROV OWN/CTRL field entry is Y or U. Worker is not able to approve the service agreement when exception 401 is posted.
