Provider attestation toolkit available for customized living and adult day services

DHS added a provider attestation toolkit for customized living and adult day services to the Transition plan for home and community-based settings webpage. Please click on “provider attestation” to find toolkit information.

The attestation process is required for providers of specific HCBS waiver services to demonstrate that they meet new federal requirements or to indicate that they need more assistance. Providers must complete this process and comply with all new setting guidelines by Sept. 1, 2017, unless you request an extension during the attestation process.

Training Opportunities
