Unbundling project

Effective Feb. 1, 2025, DHS will start the rolling implementation of the CDCS unbundling project. The current CDCS policy and CDCS unbundling project policy will be in effect at the same time. For more information, refer to the Dec. 10, 2024, eList announcement.

Definition

CDCS support planning services: Services that help a CDCS participant develop and implement his/her CDCS Community Support Plan (CSP). CDCS support planning services are a subsection of CDCS Manual – Self-direction support activities.

Overview

CDCS support planning services are optional and included in the person’s budget. When selected, support planning services are provided by certified CDCS support planners. For information about the support planner’s role, see CDCS Manual – Support planners.

Covered services

CDCS support planning services include tasks outlined in the written work agreement between the support planner and the person.

Tasks could include, but are not limited to:

  • · Providing information about CDCS and provider options.
  • · Applying person-centered thinking and planning principles to facilitate the development of a person-centered CSP.
  • · Developing a quality CSP that includes all required components and information required to authorize CDCS services.
  • · Ensuring the CSP is developed based on assessed needs identified in the person’s assessment.
  • · Submitting the CSP to the lead agency for approval.
  • · Implementing, monitoring and evaluating the approved CSP and budget on an ongoing basis.
  • · Modifying the CSP as needed, including revisions and addendums.
  • · Helping and teaching the person to recruit, screen, hire, train, schedule and monitor workers.
  • · Providing information about community resources related to the CSP.
