Overview

This guide explains how Special Needs BasicCare (SNBC) care coordinators enter health risk assessments into the Medicaid Management Information System (MMIS). SNBC uses Health Risk Assessment Screening Document – MSC+, MSHO and SNBC, DHS-3427H (PDF).

Document number (DOCUMENT NBR)

MMIS assigns an 11-digit authorization number to the screening document and automatically populates the authorization number field.

MMIS brings forward some information from the person’s previous LTC screening document and displays it on a new LTC screening document. Managed care organization (MCO) staff should enter required fields and update MMIS-populated fields as needed.

Date of birth (DOB)

Enter the person’s date of birth in MMDDYYYY format.

Activity type (ACTIVITY TYPE)

Enter the two-digit code for the activity type:

  • · 01 – Telephone screen/mailed survey: This code indicates a health risk assessment completed by telephone or mail.
  • · 02 – Face-to-face assessment visit: This code indicates an in-person health risk assessment.
  • · 05 – Care coordinator change: This code indicates the person's care coordinator has changed.
  • · 07 – Administrative activity code: This code indicates the person refused a health risk assessment, cannot be found or had a review of their last health risk assessment due to a change in product.
  • Activity date (ACT DT)

    Enter the date staff conducted the activity in MMDDYY format.

    County of service, county of residence and county of financial responsibility (COS, COR and CFR)

    Enter the three-digit county or tribal code in the following fields:

  • · County of service (COS).
  • · County of residence (COR).
  • · County of financial responsibility (CFR).
  • MMIS will correct any entries that do not match existing data in the system.

    Long-term care consultation (LTCC CTY)

    Enter the health plan code in the long-term care consultation county (LTCC CTY) field. The health plan code must match the health plan assigned on the RPPH screen in the person's file. If the code does not match, MMIS posts exception code 909 – PPHP PROVIDER NUMBER INVALID.

    Care coordinator provider number (UMPI NBR)

    Enter the care coordinator’s Unique Minnesota Provider Index (UMPI) number. All SNBC care coordinators must have a provider ID number and a provider type of 27.

    To look up a provider number in MMIS:
    1. Press function key F5 to navigate to the MMIS PROV RECIP KEY – CPRK panel.
    2. Type the provider’s name in the PROVIDER NAME LOOKUP, ENTER FULL OR PARTIAL NAME IN LAST, FIRST, MI SEQUENCE field.
    3. Press function key F4 to navigate to the MMIS PROVIDER LIST – PINQ panel in the provider file.
    4. Press F8 to advance to the next page of the list, if necessary.
    5. Press F7 to return to the previous page of the list, if necessary.
    6. Locate the correct provider and copy the UMPI number.
    7. Press function key F6 to return to the screening document.
    8. Enter the provider number in the PROV NBR field.

    Navigation

    Press a transmit/enter key to advance to the ALT2 panel.
