Overview | This guide explains how Special Needs BasicCare (SNBC) care coordinators enter health risk assessments into the Medicaid Management Information System (MMIS). SNBC uses Health Risk Assessment Screening Document – MSC+, MSHO and SNBC, DHS-3427H (PDF). Do not complete any fields on the ALT2 panel when the person refuses a health risk assessment, has a review of the last health risk assessment or changes care coordinators. | ||
Reasons for referral (REASONS FOR REF) | Keep this field blank for a health risk assessment. | ||
Current living arrangement (CURRENT LA) | Enter the two-digit code for the person’s current living arrangement. | ||
Assessment team | Enter the two-digit code for the assessment team type (02 or 03). | ||
Current housing type (CURR HOUSING) | Enter the two-digit code for the person’s current housing arrangement. | ||
Do you have any current dental concerns? | Enter yes (Y), no (N) or chose not to answer (C). | ||
Do you have a dentist? | Enter yes (Y), no (N) or chose not to answer (C). | ||
