Enter a long-term care (LTC) screening document

You must enter an LTC screening document into MMIS after a person’s assessment. This page includes instructions to:
Instructions for a person with no previous LTC screening document saved in MMIS
If the person has no assessment history and/or no LTC screening history in MMIS, you must enter a new LTC screening document.
In general, a screening document is based on a MnCHOICES assessment. When you are entering the person’s first LTC screening document, you must complete most MMIS fields. For detailed information about the fields, refer to MMIS LTSS Manual – LTC screening document screens and MMIS LTSS Manual – LTC screening document fields.
To enter a new LTC screening document:
1. Type X next to screenings on the main menu, then press the transmit key to open the ASCR key panel screen.
2. Complete the SCRNG screen and use:
3. Press the transmit key to open a new LTC screening document at the ALT1 screen.
4. Complete the ALT1 screen.
5. Press the transmit key to move to the ALT2 screen and complete it.
6. Press the transmit key to move to the ALT3 screen and complete it.
7. Press the transmit key to move to the ALT4 screen and complete it.
8. Press the transmit key to move to the ALT5 screen and complete it.
9. Press the transmit key to move to the ALT6 screen, which only appears if the PROGRAM TYPE on ALT4 is Alternative Care (AC) or Essential Community Supports (ECS) (program type 09, 10, 20, 29 or 30). Complete the ALT6 screen for AC or ECS. The ALT6 screen will not appear for other program types.
10. Add comments as needed to the COMMENTS screen. For instructions, refer to the comment screens section of MMIS LTSS Manual – LTC screening document screens.
11. Press F9 to display exception codes. For more information, refer to MMIS LTSS Manual – Exception codes in the LTC screening document.
12. Resolve exception codes.
13. Press F3 to save the document. This step is important. You must save the document even if exception codes remain. MMIS will save the completed data and allow you to return to the document for additional work.
14. Press F3 key to return to the SCRNG key panel.
15. Press F3 to continue to log out of MMIS.
Instructions for a person who has an existing LTC screening document
When a person has saved screening documents within MMIS, you will always work on a copy of the most recently saved document when you use action code A (add) or C (change) to create a new screening document for the person. To work from a copy of a screening document:
1. Type X next to screenings on the main menu, then press the transmit key to open the ASCR key panel screen.

5. Press the transmit key to move to the ALT2 screen and complete it.
6. Press the transmit key to move to the ALT3 screen and complete it.
7. Press the transmit key to move to the ALT4 screen and complete it.
8. Press the transmit key to move to the ALT5 screen and complete it.
9. Press the transmit key to move to the ALT6 screen, which only appears if the PROGRAM TYPE on ALT4 is AC or ECS (program type 09, 10, 20, 29 or 30). Complete the ALT6 screen for AC or ECS. The ALT6 screen will not appear for other program types.
