Relocation service coordination – targeted case management (RSC-TCM)

RSC-TCM is a type of targeted case management available to all people on Medical Assistance (MA) to help them plan and access supports to return to the community from an institution or other setting. RSC-TCM can supplement (but cannot duplicate) required nursing facility or residential treatment discharge planning services.
CBSM – RSC-TCM includes information about eligibility, timelines, limitations, billing, provider standards and services.
This page includes instructions to complete a screening document to authorize payment for RSC-TCM. You do not complete a service agreement for RSC-TCM.
The authorization for RSC-TCM will end when any of the following happens:
Instructions to authorize RSC-TCM for people who are not on a waiver
Use activity type 04: assessment
You can authorize RSC-TCM within 12 months of an in-person assessment. If it has been more than 12 months since the last in-person assessment, the person needs a new assessment using activity type 04 to begin RSC-TCM.
To authorize RSC-TCM in this situation:
1. Complete an assessment and enter a screening document using instructions in MMIS LTSS Manual – Enter an LTC screening document.
2. Use activity type 04 on the ALT1 screen.
3. Press the transmit key to move to the ALT2 screen and use:
4. Press the transmit key to move to the ALT3 screen and complete the ALT3 screen based on the assessment.
5. Press the transmit key to move to the ALT4 screen and use:
6. Press the transmit key to move to the ALT5 screen and enter 38-F (RSC/MHM) in the service plan summary section.
7. Press F9 to display exception codes. MMIS may post exception codes based on MA eligibility, living arrangement, current housing, etc.
8. Resolve exception codes based on individual criteria and press F9 after resolving each code.
9. Press F9 again after resolving each exception code to display exception codes for the entire document.
10. Press F3 to save the document. This step is important. You must save the document even if exception codes remain. MMIS will save the completed data and allow you to return to the document for additional work.
11. Press F3 to continue to log out of MMIS.
Use activity type 07: administrative activity
If the person had an in-person assessment (activity type 02 or 04) within the last 12 months but did not open to a waiver program and later requests RSC-TCM, you can use activity type 07 to authorize RSC-TCM.
RSC-TCM claims will pay during the 365 days from the date of the last in-person assessment. You might need to complete an activity type 04 assessment if RSC-TCM services and resulting claims will continue beyond that date.
To authorize RSC-TCM in this situation:
1. Enter a screening document.
2. Use activity type 07 on the ALT1 screen and use the date the person requested RSC-TCM as the activity type date.
3. Press the transmit key to move to the ALT2 screen.

Instructions to authorize RSC-TCM for people on a waiver
A person cannot be open to a waiver program while receiving RSC-TCM. You must close the waiver program to authorize RCS-TCM. Closing the waiver is required even if the length of the admission would not typically require you to close it (i.e., less than 30 days) if the person wants to access RSC-TCM during that time period.
To close the waiver program and authorize RSC-TCM:
1. Close the service agreement by following instructions on MMIS LTSS Manual – Close a service agreement.
2. Enter a screening document.
3. Use activity type 07 on the ALT1 screen and use the date of admission to the facility or residential treatment setting as the activity type date.
4. Press the transmit key three times to move to the ALT4 screen and use:

6. Press F9 to display exception codes.

CBSM – Temporary waiver exits and restarts: MMIS actions includes instructions for the screening document and service agreement process for admissions to different types of institutions and residential treatment settings and the return to the community from various settings.
RSC-TCM through a managed care organization (MCO)
The instructions for RSC-TCM through an MCO vary based on whether the person is using the Elderly Waiver (EW).
For people not using EW who need relocation assistance before discharge, the care coordinator enters a screening document using an activity type 04 with assessment result 18 and program type 19.
For people using EW, CBSM – Temporary waiver exits and restarts: MMIS actions provides information about closing EW when a nursing facility or residential treatment stay exceeds 29 days and completing an administrative restart of EW if the person is discharged within 121 days of admission. Care coordinators help with needed relocation activity during this time, but they do not enter a screening document.
