Service agreements and provider enrollment

Lead agencies use the service agreement to authorize services included in a person’s support plan. Information in the MMIS provider file application (also called the provider subsystem) supports this authorization and controls:

  • · Providers the lead agency can use when authorizing services on each service agreement type.
  • · Service(s) each provider can be authorized to deliver.
  • All providers who deliver services to people on Minnesota Health Care Programs (MHCP) must enroll through the Minnesota Provider Screening and Enrollment Portal. Provider enrollment records establish:

  • · Categories and types of services the provider is qualified to deliver.
  • · Enrollment status (e.g., active, pending, terminated).
  • · Dates of active enrollment.
  • · Other important information related to the provider and services.
