Home care nursing (HCN) shared services authorization process

HCN has a shared service option for services provided in the same setting, at the same time and by the same worker for people who have an agreement to share HCN services.

This page includes MMIS instructions for shared HCN. For additional steps to complete a service agreement, refer to MMIS LTSS Manual – Create a new service agreement and MMIS LTSS Manual – Change an existing service agreement.

Instructions for 1:1 service agreement line item

To complete the 1:1 service agreement line item (i.e., line item for services the person will not share):

· Enter a line item with the procedure code T1002 for RN or T1003 for LPN, the rate and the total number of units.

· Use modifier UC for extended HCN.

· Leave the SHR field blank.

· Enter 1 in the frequency field because HCN is authorized for daily use.

· Add the ordering provider’s national provider identifier (NPI) number in the ORDER NPI field.

Instructions for 1:2 service agreement line item

To complete the 1:2 service agreement line item (i.e., line item for services the person will share):

· Enter a line item with the procedure code T1002 for RN or T1003 for LPN, the rate and the total number of units.

· Use modifier TT.

· Use modifier UC for extended HCN.

· Add Y to the SHR field.

· Enter 1 in the frequency field because HCN is authorized for daily use.

· Add the ordering provider’s NPI number in the ORDER NPI field.

Exception codes

MMIS will post the following exception codes if the information you enter is not complete or correct:

· Exception code 363: The modifier is incorrect, or you used modifier TT but the SHR field is not Y.

· Exception code 376: The SHR field is not Y.

· Exception code 869: The SHR field is not Y, and you used modifier TT.

· Exception code 153: The ordering NPI field is missing or invalid.

· Exception code 639: The service agreement includes line items for extended HCN but does not also include approved line items for non-extended HCN (i.e. “state plan”) service.
