Overview

In total, there are 28 variables and a constant used in the consumer directed community supports (CDCS) budget methodology for the Developmental Disabilities (DD) Waiver. All of the variables are data elements reported on or derived from DD Screening Document, DHS-3067.

The following sections identify each variable/constant and:

  • · Variable/constant’s assigned weight used in the formula
  • · Range of scores a person may receive for each variable
  • · Field on DHS-3067 in which the person’s score for that variable is recorded.
  • Note: An “unknown” or “99” on DHS-3067 always receives a score of zero (0) in the budget methodology. For variables that have a “derived field,” the lead agency supplies those values.

    Age group

    Use the following information for the “age group” variable.

    Weight: 19.432

    Range: 1-4, where:

  • · 1 = Birth to age 17.5
  • · 2 = Age 17.5 to 21
  • · 3 = Age 21 to 28.5
  • · 4 = Older than age 28.5.

    Level of support and services

    Use the following information for the “level of support and services” variable.

    Weight: 48.724

    Range: 0-4, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (accesses supports as needed)
  • · 2 = Code of 02 (requires some services; doesn't require 24-hour plan of care)
  • · 3 = Code of 03 (needs 24-hour plan of care)
  • · 4 = Code of 04 (requires 24-hour awake supervision).

    Risk status

    Use the following information for the “risk status” variable.

    Weight: -56.839

    Range: 0-4; each person receives a score of one (1) for the risk status variable.

    Profile

    The “profile” variable determines how much the state allocates to the lead agency’s budget for the slot used by the person. Use the following information for this variable.

    Weight: -4.263  

    Range: 1-4, where a score of one (1) indicates the highest need.

    Field on DHS-3067: Derived field. The lead agency locates the score in the DD Waiver Management System.

    DT&H service authorization level

    The “day training and habilitation (DT&H) service authorization level” variable reflects the level of supervision the person needs during the day based on their assessed needs, regardless of whether they receive DT&H services. Use the following information for this variable.

    Weight: -8.737

    Range: 1-3, where a score of one (1) requires the most intensive assistance.

    Field on DHS-3067: Derived field. The lead agency locates the score in the DD Waiver Management System.

    Note: The “age group” variable accounts for CDCS budget increases when a person accesses DT&H services.

    Medical

    Use the following information for the “medical” variable.

    Weight: 9.934

    Range: 0-5, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (no serious/specialized medical needs)
  • · 2 = Code of 02 (needs specialized or frequent medical attention – office visits only, not on-site attention)
  • · 3 = Code of 03 (needs on-call medical attention)
  • · 4 = Code of 04 (needs on-site medical attention, but less than 24-hours/day)
  • · 5 = Code of 05 (needs on-site medical attention 24-hours/day).

    Mobility

    Use the following information for the “mobility” variable.

    Weight: 4.780

    Range: 0-8, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (no impairment)
  • · 2 = Code of 02 (walks short distances independently)
  • · 3 = Code of 03 (walks aided – e.g., walker, crutches, assistance of a person)
  • · 4 = Code of 04 (propels own wheelchair – bears weight for transfers)
  • · 5 = Code of 05 (propels own wheelchair – total assistance with transfers)
  • · 6 = Code of 06 (uses electric wheelchair)
  • · 7 = Code of 07 (unable to propel wheelchair)
  • · 8 = Code of 08 (not mobile due to overriding medical conditions).

    Mental health services

    The “mental health services” variable indicates if the person’s support plan should include mental health services. Use the following information for this variable.

    Weight: 14.358

    Range: 0 or 1, where:

  • · 0 = No (N)
  • · 1 = Yes (Y).

    Self-preservation

    Use the following information for the “self-preservation” variable.

    Weight: 5.973

    Range: 0-3, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (is capable of self-preservation)
  • · 2 = Code of 02 (requires verbal/physical prompts for preservation)
  • · 3 = Code of 03 (is not capable of self-preservation).

    Psychosis

    The “psychosis” variable indicates if the person has a pervasive developmental disorder diagnosis, per the International Statistical Classification of Diseases and Related Health Problems (ICD). Use the following information for this variable.

    Weight: 35.518

    Range: 0 or 1, where:

  • · 0 = Fields 12, 13, 14 or 15 do not include an ICD code that begins with F84
  • · 1 = Include ICD code that begins with F84.

    Cerebral palsy

    The “cerebral palsy” variable indicates if the person has an infantile cerebral palsy diagnosis, per the ICD. Use the following information for this variable.

    Weight: 8.394

    Range: 0 or 1, where:

  • · 0 = Fields 12, 13, 14 or 15 do not include an ICD code that begins with G80
  • · 1 = Include ICD code that begins with G80.

    Epilepsy

    The “epilepsy” variable indicates if the person has an epilepsy and recurrent seizures diagnosis, per the ICD. Use the following information for this variable.

    Weight: 7.004

    Range: 0 or 1, where:

  • · 0 = Fields 12, 13, 14 or 15 do not include an ICD code that begins with G40
  • · 1 = Include ICD code that begins with G40.

    Seizures

    Use the following information for the “seizures” variable.

    Weight: 2.151

    Range: 0-5, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (no history or evidence of seizures)
  • · 2 = Code of 02 (history of seizures; none recently)
  • · 3 = Code of 03 (seizures – controlled)
  • · 4 = Code of 04 (seizures – partially controlled)
  • · 5 = Code of 05 (seizures – uncontrolled).

    Level of developmental disabilities

    The “level of developmental disabilities” variable indicates if and to what degree the person has a developmental disability diagnosis, per the ICD. Use the following information for this variable.

    Weight: 5.128

    Range: 1-4, where:

  • · 1 = Fields 12, 13, 14 or 15 include ICD code F70 or F79
  • · 2 = Include ICD code F71
  • · 3 = Include ICD code F72
  • · 4 = Include ICD code F73.

    Related conditions

    The “related conditions” variable indicates if the person has a related condition diagnosis, per the ICD and Minnesota statutes and regulations. Use the following information for this variable.

    Weight: 13.063

    Range: 0 or 1, where:

  • · 0 = Fields 12, 13, 14 or 15 do not include ICD-code F78
  • · 1 = Include ICD code F78.
  • Field on DHS-3067: 12-15

    Vocational

    Use the following information for the “vocational” variable.

    Weight: -1.481

    Range: 0-6, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 05 (needs intensive support – with or without adaptations)
  • · 2 = Code of 04 (needs moderate support – with or without adaptations)
  • · 3 = Code of 03 (needs minimal support – with or without adaptations)
  • · 4 = Code of 02 (needs on-the-job training – time limited)
  • · 5 = Code of 01 (independent – requires typical training; may use adaptations)
  • · 6 = Code of 09 (N/A).

    Leisure and recreation

    Use the following information for the “leisure and recreation” variable.

    Weight: 2.590

    Range: 0-5, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (independent)
  • · 2 = Code of 02 (minimal supervision – formal program not needed)
  • · 3 = Code of 03 (instruction required with expected outcome of increased independence)
  • · 4 = Code of 04 (person participates with another's assistance for all or portions of an activity)
  • · 5 = Code of 05 (person unable to participate in activity).

    Occupational therapy

    The “occupational therapy” variable indicates if the person’s support plan should include occupational therapy. Use the following information for this variable.

  • · 0 = No (N)
  • · 1 = Yes (Y).

    Community living

    Use the following information for the “community living” variable.

    Weight: 3.248

    Range: 0-5, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (independent).

  • · 3 = Code of 03 (instruction required with expected outcome of increased independence)

    Expressive communication

    Use the following information for the “expressive communication” variable.

    Range: 0-8, where:

  • · 0 = Code of 99 (unknown)
  • · 1 = Code of 01 (functional)
  • · 2 = Code of 02 (speech intelligible to familiar listeners)
  • · 3 = Code of 03 (speech difficult to understand)
  • · 4 = Code of 04 (speech unintelligible even to familiar listeners)
  • · 5 = Code of 05 (combines signs and/or gestures to communicate)
  • · 6 = Code of 06 (uses single signs or gestures to express wants and needs)
  • · 7 = Code of 07 (uses augmentative communication aid)
  • · 8 = Code of 08 (does not have functional expressive communication).

    Constant

    The constant is -120.534.

    It is applied to every person and based on the actual annual waiver and home care costs of people who do not use CDCS. The sum of the 28 variables consistently exceeds the actual amounts spent. The statistical modeling anticipates this and generates the constant to ensure a neutral starting point.
