Unbundling project

This page includes CDCS unbundled service category policy. When a person transitions to the unbundled CDCS service categories, they will use the policy on this page.

For information about who must use this new policy, refer to the Dec. 10, 2024, eList announcement.

Definitions

Treatment and training: CDCS service category that promotes the person’s health and ability to live and participate in the community.

Specialized therapies and behavioral supports: Services a Minnesota Health Care Program (MHCP) medical provider prescribes to relieve the person’s disability (all waivers and AC) and/or health condition (AC/EW).

Training and education for paid and unpaid caregivers: Services that pay for training and education to support the caregiver’s ability to care for the person.

Training and education for the person: Services that pay for training and education to increase the person’s ability to manage their CDCS services.

Class: A single training.

Course: A series of classes.

Conference: A single or multi-day workshop or training, annual conference or symposium.

This page includes policy and procedure about the CDCS treatment and training service category that covers:

    Specialized therapies and behavioral supports

    Covered services

    Specialized therapies and behavioral supports cover services that meet all the following criteria:

  • · Follow the policies on CDCS Manual – Allowable and unallowable goods and services under CDCS.
  • · Not available through other funding sources, such as Medical Assistance (MA).
  • · Relieve the person’s disability and/or health condition.
  • · Prescribed by an MHCP medical provider licensed to practice in Minnesota (physician, physician’s assistant or advanced practice registered nurse [APRN]).
  • Additional documentation

    The person’s medical provider must complete and sign CDCS Specialized Therapy Request Form, DHS-5788C and/or CDCS Behavioral Support Request Form, DHS-5788B. The lead agency reviews the form(s) with the person’s assessment and CDCS community support plan to determine whether the requested specialized therapy or behavioral support meets all required criteria listed above.

    Non-covered services

    Specialized therapies and supports do not cover:

  • · Services that do not meet the criteria on CDCS Manual – Allowable and unallowable goods and services under CDCS.
  • · Services covered by MHCP. For directions about services available through MHCP, refer to MHCP Provider Manual.
  • · Traditional waiver/AC services. For directions about purchasing services through traditional waiver/AC, refer to CDCS Manual – Purchasing traditional waiver/AC goods and services under CDCS.
  • · Items or goods.
  • · Experimental treatments, as defined under Minn. R. 9525.3015, subp. 16.
  • · Services that exceed current MA coverage limits.
  • Training and education for paid and unpaid caregivers

    Covered services

    Training and education for paid and unpaid caregivers covers the cost of training registration fees and materials for paid and unpaid caregivers to support their ability to care for the person. The training and education must meet all the following criteria:

  • · Follow the policies on CDCS Manual – Allowable and unallowable goods and services under CDCS.
  • · Help the caregiver provide the person’s care or support (e.g., apply person-centered principles, learn and implement techniques and strategies to support the person, learn about interventions and supports, learn to use equipment).
  • Non-covered services

    Training and education for paid and unpaid caregivers does not cover:

  • · Services that do not meet the criteria on CDCS Manual – Allowable and unallowable goods and services under CDCS.
  • · Expenses for travel.
  • · Lodging or meals related to attending classes.
  • Note: The person pays for the time a paid caregiver spends attending training from the personal assistance or community integration service categories.

    Training and education for the person

    Covered services

    Training and education for the person covers the cost of training registration fees and materials for the person to increase their ability to manage their CDCS services. The focus of the training must be at least one of the following topics:

  • · Develop self-advocacy skills.
  • · Exercise civil rights.
  • · Acquire skills that enable the person to exercise control and responsibility over the supports they receive.
  • · Apply person-centered principles (refer to DHS – Person-centered practices).
  • · Learn about interventions and supports.
  • · Use equipment.
  • The training and education must follow the policies on CDCS Manual – Allowable and unallowable goods and services under CDCS.

    Non-covered services

    Training and education for the person does not cover:

  • · Services that do not meet the criteria on CDCS Manual – Allowable and unallowable goods and services under CDCS.
  • · Expenses for travel, lodging or meals related to attending classes, conferences and courses.
  • · Payment to the person to attend classes, conferences and courses.
  • · Ongoing skills training covered by other CDCS service categories.
  • · 1:1 training for the person in their home.
  • Remote support service delivery option

    CDCS treatment and training can be delivered through remote support. Services delivered through remote support must meet all the requirements listed on CBSM – Remote support.

    Provider standards and qualifications

    Specialized therapies and behavioral supports

    Specialized therapies or behavioral supports providers must:

  • · Meet the certification or licensing requirements in state law related to the service being provided (e.g., hippotherapy from a provider certified by the American Hippotherapy Certification Board or occupational therapy, physical therapy or speech-language therapy from a board-certified provider).
  • · Be included in the person’s CDCS community support plan.
  • Training and education for paid and non-paid caregivers and the person

    Training and education providers must hold relevant professional licenses, credentials or certifications as required by state law.

    For training and education that does not require professional licenses, credentials or certifications, the person can define provider qualifications in their CDCS community support plan.

    Lead agency responsibilities

    Before the person receives services, the case manager must:

  • · Verify the provider holds the license, credential or certification required by state law for the service.
  • · Ensure the provider has not had state or county agency contracts or provider agreements discontinued due to fraud.
  • · Ensure the provider has not been disqualified under the criminal background check according to the standards in Minn. Stat. Ch. 245C.03 (Background Studies Act).
  • If state law does not require the provider of the service to hold a license, credential or certification, the case manager is responsible to verify the provider meets the qualifications defined in the CDCS community support plan. The person must maintain documentation of provider qualifications.
