Definition | Local system needs plan (LSNP) amendment: Determination of a change in the need for services from an intermediate care facility for persons with developmental disabilities (ICF/DD) after submission of the two-year planning document. | ||
Background | The LSNP amendment allows counties to change submitted LSNPs based on assessments and identification of ICF/DD service needs within counties/regions by program type, location, demographics and size of the licensed services for persons with developmental disabilities or related conditions. Revise LSNPs in collaboration with ICF/DDs based on choices and needs of the people who reside there. The amended plans demonstrate how changes to specific ICFs/DD fit into the overall service delivery model within the counties and regions. | ||
Timelines | Amend local system needs plans anytime: Modifications to existing services may include conversion from Class A to Class B licensure, change in ages served, closures, downsizing or relocation of services. | ||
Documentation | Closures, downsizings and relocations require county submission of information on DHS forms. Amended LSNPs are required for submission of intended county actions that amend existing two-year plans. Complete Local System Needs Plan Amendment DHS 4677C (PDF). | ||
Submission | Submit plans and documentation at least 90 working days before anticipated changes. Include supporting documentation such as closure, downsizing and/or relocation plans with the amendments. | ||
Policies and procedures | DHS staff review amendments to assess compliance with applicable federal and state laws and policies. If DHS approves the LSNP amendment, DHS notifies the county, Licensing Division and Department of Health in writing. | ||
