Definition | ICF/DD relocation: Plan to move licensed and certified beds from an existing ICF/DD to an alternative location resulting in downsizing of the original facility and creation of a new ICF/DD. | ||
County support | The host county in which the ICF/DD is located plays the lead role in the relocation process. The county reviews the current local system needs plan (LSNP) and determines if the county supports the relocation of the ICF/DD and movement of the licensed and certified ICF/DD beds. Counties base the decision to support or not support the relocation of specific beds from the ICF/DD on the following: | ||
Facility planning | Facilities work in cooperation with the host county and county of financial responsibility (CFR) case managers to support the identified choices of each person currently living in the ICF/DD. ICFs are responsible to develop a plan that addresses all of the items identified in the relocation plan checklist. | ||
Documentation | If the county supports the relocation, they submit the following to DHS. | ||
Process | For information about the process, refer to ICF/DD Manual – Lead agency process to close relocate or downsize an ICF/DD. | ||
DHS approval | DHS staff review documentation received. DHS bases decisions on the following: DHS approves the request, denies the request or asks for additional information. Once DHS determines a final decision, DHS notifies the host county in writing. Upon approval, the host county provides a copy of the plan to the ICF/DD. The county may then move forward with the plan as written and approved. Submit any changes and/or revisions to the approved plan to DHS. This includes changes in dates or timelines. DHS must approve or disapprove any changes. Only then can changes be implemented. DHS will notify the Department of Health and DHS Licensing regarding the relocation plan approval. | ||
