Definitions | Nutrition services: Services that include nutrition education and nutrition counseling to address an older adult’s nutritional needs. The goal of these services is to improve or maintain a person’s nutritional status and improve management of his/her chronic diseases or conditions. Nutrition education: One or more individual or group sessions that provide formal and informal opportunities for people to gain knowledge and skills in managing their diet and nutritional needs. Nutrition counseling: One or more individual sessions to advise and assist people on appropriate nutritional intake. Nutritional counseling includes assessment of a person’s nutritional needs that results in an individualized plan with goals and follow-up on established goals. | ||
