Definitions

Nutrition services: Services that include nutrition education and nutrition counseling to address an older adult’s nutritional needs. The goal of these services is to improve or maintain a person’s nutritional status and improve management of his/her chronic diseases or conditions.

Nutrition education: One or more individual or group sessions that provide formal and informal opportunities for people to gain knowledge and skills in managing their diet and nutritional needs.

Nutrition counseling: One or more individual sessions to advise and assist people on appropriate nutritional intake. Nutritional counseling includes assessment of a person’s nutritional needs that results in an individualized plan with goals and follow-up on established goals.

Nutrition education

Covered nutrition education sessions include:

  • · Shopping
  • · Food selection
  • · Meal preparation
  • · Menu planning
  • · Preparing normal and therapeutic diets
  • · Cooking for one or two people
  • · Tips for eating well on a limited budget.
  • Nutrition counseling

    Covered nutrition counseling sessions include:

  • · Assessment of a person’s nutritional needs that results in an individualized plan with goals
  • · Follow-up on established nutritional goals.
  • Nutrition counseling can include helping people with:

  • · Therapeutic diets (e.g., diabetic, low sodium, low cholesterol, renal or gluten-free diets)
  • · Weight management strategies for people who are chronically underweight or overweight
  • · Severe weight loss or gain
  • · Difficulty chewing or swallowing
  • · Other nutritional care issues.
  • Secondary information

    Nutrition services are tied to a specific goal and authorized in the older adult’s community support plan. Providers must deliver services that are consistent with the person’s cultural background.

    Provider standards and qualifications

    Professionals qualified to provide nutritional services include:

  • · Licensed dietitians and licensed nutritionists under Minnesota Statutes, Section 148.621 and Minnesota Rules, Chapter 3250
  • · Registered dietitians who meet education and practice requirements specified for licensed dieticians and nutritionists
  • · Other professionals who are exempt from licensure, according to Minnesota Statutes, Section 148.632, and perform service incidental to their practice (e.g., diabetic nurse practitioners when nutrition practice is related to their practice, registered nurses).
  • Authorization, rates and billing

    The lead agency authorizes nutrition services provided on a per-visit basis.
