Activity timelines for reassessments when the COR and CFR are different

This table provides an overview of the timelines for case manager and certified assessor activities during annual reassessments for people on the BI, CAC, CADI and DD waivers and people who receive Rule 185 case management. These activity timelines apply when the county of residence (COR) and county of financial responsibility (CFR) are different.

To use the table, start with the row associated with the person’s service agreement (SA) end date.

CM reviews current support plan with person

CM notifies COR of needed reassessment / COR processes assignments

CA schedules reassessment and prepares for visit

CA completes reassessment and CSP

CM completes CSSP and SA
