Definitions

Home care nursing (HCN): Professional nursing services provided to a person who requires more continuous care than a skilled nursing visit (SNV) and beyond the scope of what personal care assistance (PCA)/Community First Services and Supports (CFSS) and home health aide (HHA) services can provide. HCN services are based on an assessment of the person’s medical/health care needs and are provided in the person’s place of residence or in the community. All HCN services must be ordered by a physician, physician assistant (PA) or advanced practice registered nurse (APRN) every 60 days.

Shared service option for HCN: HCN services provided:

  • · In the same setting at the same time and by the same worker.
  • · To two people eligible for HCN who agree to share HCN services.
  • Covered services

    Shared HCN services must meet all the following criteria:

  • · Covered and authorized as described on CBSM – HCN.
  • · Delivered in the same setting at the same time and by the same worker.
  • · Delivered to two people eligible for HCN as described on CBSM – HCN.
  • · Delivered to two people who sign Home Care Shared Services Agreement (HCN, PCA or CFSS), DHS-6893E.
  • Non-covered services

    Shared HCN services are not covered if they are:

  • · Non-covered or non-authorized HCN services as described on CBSM – HCN.
  • · Delivered to more than two people who receive HCN services from one nurse at the same time.
  • · Delivered to people who have not signed Home Care Shared Services Agreement (HCN, PCA or CFSS), DHS-6893E (PDF).
  • Rates

    Shared service payment rates apply if a nurse cares for more than one person in one setting at the same time. Only one nurse can provide services at the same time for people who choose the shared services option in the same setting.

    The person may choose to stop sharing services at any time. Choosing to start or stop sharing HCN services does not result in the person getting extra HCN units.

    Case manager responsibilities

    If the person is on a waiver or Alternative Care (AC), their case manager is responsible to provide them with information about the shared service option.

    Provider responsibilities

    The HCN provider must:

    1. Ensure the shared service option is appropriate based on people’s ages, compatibility and coordination of assessed care needs.

    2. Offer people the option of shared services, one-to-one services or a combination of both.

    3. Complete Home Care Shared Services Agreement (HCN, PCA or CFSS), DHS-6893E (PDF) for both people who share HCN services and list the expected outcomes.

    4. Ask all people who share HCN services to sign DHS-6893E and keep it in the agency file.
