Background
Using feedback gathered in December 2018, DHS and partners have been working to update the Positive Support Transition Plan and related documents.
We took that feedback and updated the forms/instructions to make them more effective and user friendly. Some of the updates in the new drafts include:
Purpose of PSTP process
Positive Support Transition Plans help service providers phase out the use of prohibited or restricted procedures, such as manual or mechanical restraint. Plans are designed for support providers to use as an implementation guide for positive support strategies (i.e., strengths-based strategies that emphasize teaching people productive and self-determined skills or behaviors). The review form is used to assess how successful the strategies are within the plan and to determine if changes are needed.

NOTE: The previous versions of the PSTP plan and review forms are dynamic PDFs. It can be hard to open dynamic PDFs in Google Chrome or on a smart phone or tablet.
