Clarification about how to use the DSD Resource Center Portal, DHS-3754

In an Oct. 1, 2020, eList announcement, DHS announced a secure form for lead agencies to use when contacting the DSD Resource Center. This announcement provides clarification about when to use the new form.

When to use DHS-3754

Lead agencies must use DSD Resource Center Portal, DHS-3754 when they need assistance with person-specific issues related to service agreement change requests, edit questions and MMIS issues for:

· Alternative Care (AC) and Essential Community Supports (ECS) programs

· Brain Injury (BI) Waiver, Community Alternative Care (CAC) Waiver, Community Access for Disability Inclusion (CADI) Waiver, Developmental Disabilities (DD) Waiver and Elderly Waiver (EW)

· Assessment and screening documents, including long-term care consultation (LTCC), preadmission screening (PAS), DD screening and Omnibus Budget Reconciliation Act (OBRA) screening.

The secure form eliminates the need for lead agencies to send a secure email to the Resource Center.

When not to use DHS-3754

Lead agencies should not use DHS-3754 for screening document deletions or technical change requests for home care, personal care assistance (PCA), home health aide (HHA), skilled nursing visit (SNV) and home care nursing (HCN).

Lead agencies must continue to fax the following documents to the Resource Center at 651-431-7447:

· MA Home Care Technical Change Request, DHS-4074 (PDF)

· Screening Deletion Request, DHS-4689A (PDF)

· Housing Access Coordination Exception Request for People with Criminal, Subpar Credit or Eviction History, DHS-6218 (PDF)

· Early Intensive Developmental and Behavioral Intervention (EIDBI) Technical Change Request, DHS-6516 (PDF).
